Job SummaryWe are seeking to recruit a Health Care Practitioner specialising in Older People to join our friendly team in the lovely town of Sherborne.
You will be joining our existing Frailty Nurse providing a proactive role within the Sherborne Area PCN MDT, to identify and support the holistic assessment of our elderly patients. Working collaboratively to provide the best care for frail older adults, helping them to live independently in their own homes.
With a caring and compassionate outlook, you will be responsible for delivering effective comprehensive geriatric assessment. Compiling Personalised Care Plans, helping patients to manage their long-term conditions and general health and wellbeing, and supporting patients discharged from secondary care.
Applications are invited from experienced Healthcare Practitioners who are able to demonstrate enthusiasm and a clear patient centred approach to promoting independence and wellbeing. You should be able to prioritise and manage your own caseload and work closely with other members of our wellbeing team.
Although based at The Grove Medical Centre, your role will involve meeting patients in their own homes and possibly working from our other two practices: The Apples Medical Centre and Yetminster Health Centre
This is a part-time position, for 30 hours per week. There is a degree of flexibility regarding days and hours worked.
A current driving licence and vehicle will be required
Main duties of the jobWorking closely with GPs, nursing teams and other staff of the MDT across three practices, and in the community. Supporting vulnerable older adults and their families, acting as the link between the MDT, patients, and their relatives and carers.
Utilising excellent communication skills to manage a case load of patients, promote collaborative working, and adapt to an ever-changing environment.
The post holder will use specialist knowledge and skills to support the delivery of specialist nursing care, to help create an educative and supportive framework for geriatric care.
About usSherborne Area Primary Care Network consists of three local practices, The Grove Medical Centre, The Apples Medical Centre and Yetminster Health Centre, all working together for the benefit of our local community. The PCN team allows us to provide a variety of specialist services to our patients, including a Pharmacy Team, Social Prescribers, Frailty Clinicians and Mental Health Practitioners. The successful candidate will probably work in only one or two practices, but will be a key role within the PCN team. Engaging with local services in the community, helping to continue to provide a high level of care to all our patients.
Job responsibilitiesWorking alongside our established Frailty Nurse, the PCN Clinical Director and GP Lead, to support the PCN with the continued development of our frailty service.
Planning and providing anticipatory and current care for identified patients from one or two practices in the PCN. Including treatment, preventative care, screening, future planning, and patient education. Usually in the patients home, in the practice, in one of our local care homes, or whilst working at an outreach or PCN event
To act as a focal point of communication between patients and members of our primary health care team, Dorset Healthcare, and other services in the community.
Establish a collaborative working relationship with the Care Homes allocated to the PCN, colleagues within Secondary Care, and other aligned services.
Manage own case load, working independently and as part of the PCN wide team, continuing to deliver excellent healthcare to our 25,000 patients.
Person SpecificationQualificationsEssential- Registered General Nurse Qualifications/Certificates or equivalent, in wound management, adult vaccinations and immunisations, phlebotomy.
- A demonstrable commitment to professional development.
Desirable- Qualifications/Certificates in the management of one or more long term conditions.
ExperienceEssential- Experience of working in accordance with the NMC Code of Professional Conduct (2002) and The Scope of Professional Practice (1992) or equivalent
- Experience of Microsoft Office software
- Experience of dealing with the public/patients
- Completing personalised care plans
Desirable- Experience of TPP SystmOne clinical system and Ardens
- Experience of working in primary care within an MDT
- Liaising with local organisations and other agencies
- Providing peer to peer support
