Job summaryWe are seeking a motivated, compassionate and experienced Registered Mental Health Nurse (RMN) to join our GP practice in a new part-time role, working three days per week.
This is an exciting opportunity for a mental health professional to help develop and deliver high-quality, patient-centred mental health care within a primary care setting. The successful candidate will work closely with our GPs, practice nursing team, administrative colleagues and external mental health services to improve access to support and promote positive outcomes for patients.
The role will have a particular focus on supporting patients with severe mental illness (SMI), undertaking structured mental health reviews, developing and reviewing care plans, supporting medication-related monitoring and working collaboratively with other professionals involved in patients' care.
We are looking for someone who is approachable, organised and confident working with patients experiencing a range of mental health difficulties. The successful candidate will be able to work autonomously within their professional competence while recognising when advice, supervision or escalation is required.
Previous experience in any relevant mental health setting will be considered. Experience of working within primary care would be beneficial but is not essential, as support will be provided to help the successful candidate develop within the role.
Main duties of the jobThe successful applicant will:
- Undertake structured mental health reviews for patients with severe mental illness, ensuring appropriate documentation, follow-up and onward referrals.
- Carry out mental health assessments within their competence, identifying changes in presentation, clinical concerns and potential risks.
- Develop, implement and review individualised mental health care plans in collaboration with patients, carers where appropriate, GPs and relevant services.
- Support appropriate medication monitoring and medication reviews in collaboration with the prescribing clinician.
- Identify safeguarding concerns and follow practice policies and statutory requirements.
- Provide support and signposting for patients experiencing substance misuse difficulties, working with specialist services where appropriate.
- Liaise with community mental health teams, psychiatrists, social care, substance misuse services and other relevant agencies.
- Promote continuity of care, self-management, recovery and patient engagement.
- Maintain accurate, timely and confidential clinical records.
- Contribute to the ongoing development of mental health services within the practice.
