Job summaryThe post holder will be responsible for maintaining accurate, complete and up-to-date patient records, ensuring that relevant clinical information is appropriately recorded and coded within the practice clinical system. The role will involve working closely with the wider practice team to ensure that patient information is processed efficiently and accurately.
Main duties of the job- Reviewing new and existing patients medical records and inputting, updating, and tidying an accurate summary of the patients medical history.
- Read coding a variety of information into the practice clinical system including checking medication, populating templates and coding from letters and pathology.
- Recording of A&E attendances onto patient records.
- Deal with queries relating to coding.
- Recording vaccinations.
- Identify relevant clinical information contained in patient correspondence.
- Receiving and coding a variety of patient letters via Document Inbox/tasks and forwarding on where required.
- Identifying patients with possible Access Needs and recording on the record.
- Processing NHS 111 and OOH tasks.
- Assigning pathology.
- Filing bowel screening results.

Person SpecificationQualificationsEssential- Educated to GCSE and A Level standard, or equivalent qualifications.
- Evidence of a good standard of written and verbal communication.
- Demonstrable ability to work accurately with detailed and confidential information.
ExperienceDesirable- Experience of working in General Practice
