Main duties of the job
We have an exciting opportunity for an experienced Insulin Management Diabetes Pharmacist to join the MORPh team in a flexible, remote role. This position is specifically designed for a pharmacist with advanced competency in insulin adjustment, titrations and regimen changes.

This role is open to locum candidates who meet the specific clinical requirements. We are looking for a dynamic and enthusiastic individual with a deep, practical expertise in managing complex type 2 diabetes patients on insulin, with a particular focus on optimising insulin therapy.

For this position, we require someone who doesn't just have general diabetes knowledge, but who possesses the confidence and capability to make autonomous, evidence-based insulin changes. You will use your clinical expertise to provide advanced prescribing recommendations, with a core responsibility of safe and effective changes in insulin to achieve glycemic targets.

About us
MORPh clinical services have a national team of pharmacists delivering a range of therapy review services to GP surgeries, supporting improved clinical outcomes and medicines optimisation. Project dependent, we can support both in-hours and out-of-hours working. While we deliver services across multiple therapy areas, this specific role is focused on our Diabetes portfolio with a heavy emphasis on insulin management.

Job responsibilities
Role and Responsibilities

• Insulin Optimisation: Review and adjust insulin regimens (including basal, bolus, and mixed insulins) for patients with Type 2 diabetes, based on blood glucose monitoring, HbA1c trends, and lifestyle factors.
• Treatment Escalation: Conduct comprehensive reviews where insulin initiation or complex regimen changes are required. Make appropriate recommendations to GPs and senior clinicians for insulin adjustments and adjunctive therapies.
• Medication Reviews: Undertake holistic clinical medication reviews with patients, focusing on the safety and efficacy of insulin and other glucose-lowering therapies, and produce actionable recommendations.
• Patient Follow-up: Provide structured follow-up for patients post-insulin change to monitor for hypoglycemia and overall therapeutic response.
• Clinical Enquiries: Answer any medicine-related enquiries from the patients specifically regarding insulin therapies change.
• Multidisciplinary Working: Ensure patients are referred to the appropriate healthcare professional (e.g., Specialist Nurses, Dietitians, Endocrinologists) where insulin adjustments require multidisciplinary input.

The Ideal Candidate Must Meet The Following:

• GPhC registered with a minimum of 2 years post-registration experience, with at least 1 year dedicated specifically to diabetes management.
• Degree in Pharmacy (MPharm/BPharm).
• Clinical Systems Trained: Proficient in SystemOne (S1) and EMIS.
• Insulin Competency: Proven, demonstrable experience in initiating, titrating, and prescribing insulin therapies (including analogue insulins and mixed regimens) in a primary care setting.
• Diabetes Expertise: Up-to-date knowledge of NICE guidelines and best practice for Type 2 diabetes management.
• Prescribing: Experienced in conducting diabetes treatment escalation and making clinically sound changes to insulin doses.
• Communication: Excellent communication skills to effectively counsel patients on complex insulin regimens and liaise with a wide range of healthcare professionals.
