Job summary
Join our dynamic Acute & Emergency Medicine pharmacy team and make a real impact at the front door of care. As a Band 7 Clinical Pharmacist, you'll work across the Emergency Department, Acute Medical Unit, Same Day Emergency Care (SDEC), Frailty in-reach and Urgent Treatment Centres--supporting safe, effective and timely treatment for acutely unwell patients.

You'll provide high-quality clinical pharmacy input, optimise therapies, support admission avoidance (particularly in frailty), and help facilitate safe discharge directly from ED. Working closely with the multidisciplinary team, you'll play a key role in improving patient flow and medicines safety in a fast-paced environment.

Main duties of the job
Deliver safe, effective clinical pharmacy services across ED, AMU, SDEC, Frailty in-reach and UTC.

Provide medicines optimisation during ward rounds and MDT discussions to support timely clinical decisions.

Use (or work towards) independent prescribing to optimise therapies and support admission avoidance.

Contribute to quality improvement, audit and service development within Acute & Emergency Medicine.

Support day-to-day clinical pharmacy activity, including medicines reconciliation, review and discharge planning.

Provide clear, patient-focused medicines advice to support safe discharge and virtual ward pathways.

Work with senior pharmacists to ensure safe prescribing, guideline adherence and effective medicines governance.

Job responsibilities
To provide a comprehensive clinical pharmacy service to wards and departments providing a medicines management service directly to patients with support and training from experienced staff.

To participate in the pharmacy dispensary, late night and oncall rotas as necessary.

To provide advice to medical and nursing staff on drug usage, and encourage compliance with the hospital/health economy formulary to ensure safe, rational , and economic prescribing.

To provide a professional lead to pharmacy technical and support staff managing medicines in designated ward areas.

To integrate into the multidisciplinary team and attend ward rounds making proactive interventions in individual patients therapy by providing information on drug related issues and acting as a Pharmacy contact.

To provide advice to patients on the correct use of medication and ensure that directions associated with medications are understood, including the main side-effects of the medications.

To identify drug related admissions and resolve medicines related problems associated with individual patients between primary and secondary care.

To support senior pharmacists contributing/providing pharmaceutical advice to the multidisciplinary audit of treatment guidelines.

To support senior pharmacists providing pharmaceutical advice at directorate level.

To maintain an up to date knowledge of developments in medical and pharmaceutical practice, as part of their own continuing professional development.

Qualification
Essential

• Current registration as a pharmacist with the General Pharmaceutical Council (GPhC)
• Relevant postgraduate clinical pharmacy qualification, or evidence of working towards one
• Independent prescribing qualification, or willingness and eligibility to undertake it within an agreed timeframe

Desirable

• Relevant advanced clinical assessment qualification
• Experience of implementing and delivering the objectives of cost/service improvement programmes.

Experience
Essential

• Clinical pharmacy experience across medical, surgical, urgent or emergency care specialties relevant to the post
• Experience of undertaking medicines reconciliation, medicines optimisation and discharge planning
• Excellent verbal and written communication skills
• Experience of working effectively as part of a multidisciplinary team
• Ability to manage time effectively and prioritise a variable and unpredictable clinical workload

Desirable

• Evidence of participation in service improvement projects
• Direct clinical pharmacy experience within the Emergency Department, Acute Medical Unit, Same Day Emergency Care or acute frailty services
• Experience of practising as an independent prescriber
• Experience of contributing to the development or implementation of clinical guidelines, pathways, protocols or standard operating procedures
