Job summary
Are you looking to work in a dynamic, flexible and forward-thinking team - ready to adapt and accommodate every eventuality to meet patients needs safely in the community? If so, Care Coordination is the team for you.

We have multiple workstreams which ensure variety in our work and opportunity for professional development - our drive and determination to meet the needs of our patients meant that we were HSJ award finalists in September 2022.

Main duties of the job
Working remotely, our team triage information from colleagues and members of the public in the Wolverhampton locality to ensure that we refer and signpost people to the most appropriate service to meet their needs. In summary our Care Coordination work streams include:
  • Triage over the telephone or via video consultation to support people with an acute need to avoid admission avoidance. We work closely with our Urgent Community Response Team and West Midlands Ambulance Service to achieve safe community care for acutely unwell Wolverhampton residents.
  • Triage over the telephone or via video consultation for patients with long term conditions requiring ongoing health and/or therapy support while residing in their own homes.
  • Remote triage is our main role and responsibility, but alongside this we also: Offer a see and treat face to face triage service for patients who have an unmet need and would be routinely referred to a District Nurse or Therapist, but would benefit from an assessment visit on the day of referral

In addition to the above
As a Junior Sister/Charge Nurse within the Care Coordination you will be required to provide managerial leadership support to the team, support staff in their development and lead with service innovation. Experience and confidence in utilising computers and web-based applications is essential.

  • NMC PIN
Desirable
  • Appropriate post registration course for care setting such as, Health Assessment Module

Leadership

  • Leadership experience/qualification

Experience/Skills
Essential
  • Use of databases, standard keyboard skills and knowledge of a number of IT packages such as Word, PAS, e -rostering etc
Desirable
  • Evidence of working within Community Nursing but not as a student nurse.
  • Rapid Response/Emergency Care Triage
