Job summary
The Uplands Medical Practice is seeking an experienced and motivated Practice Nurse to join our friendly and supportive multidisciplinary team for three days per week.

The successful candidate will deliver high-quality nursing care within a busy GP practice, supporting patients across a range of primary care services. The role will include chronic disease management, health promotion, immunisations, screening, and preventative care.

We are particularly interested in candidates with experience in the management of COPD, Asthma, and Diabetes, who are passionate about improving patient outcomes and promoting self-management of long-term conditions.

Main duties of the job
The post holder will be responsible for providing evidence-based nursing care and supporting the delivery of high-quality primary care services.

Job responsibilities
Clinical Responsibilities
  • Deliver Practice Nurse services within agreed competencies and and NMC standards.
  • Undertake chronic disease reviews including:
    • Diabetes
    • Asthma
    • COPD
  • Conduct annual reviews and care planning for patients with long-term conditions.
  • Administer childhood, adult, and seasonal vaccinations in line with national guidance.
  • Undertake cervical cytology screening.
  • Provide lifestyle advice and support relating to smoking cessation, weight management, and healthy living.
  • Undertake blood pressure monitoring, ECGs, venepuncture, and other nursing procedures as required.
  • Support infection prevention and control processes.
  • Maintain accurate and contemporaneous patient records using EMIS Web.
  • Participate in clinical audits and quality improvement initiatives.

Professional Responsibilities
  • Work within NMC Code of Professional Conduct.
  • Maintain professional registration and continuing professional development.
  • Participate in mandatory training and practice meetings.
  • Work collaboratively with GPs, HCAs, pharmacists, and the wider multidisciplinary team.
  • Support the achievement of Quality and Outcomes Framework (QOF) targets.

Qualifications
Essential
  • Registered General Nurse (RGN) with current NMC registration.
  • Evidence of ongoing professional development.

Desirable
  • Independent Prescriber Qualification.
  • Chronic Disease Management qualification.
  • Asthma Diploma or equivalent.
  • Diabetes Diploma or equivalent.
  • Paediatric Phlebotomy
