Job summary
We are looking for an enthusiastic and proactive Clinical Pharmacist to join our friendly and supportive practice team.

This is an exciting opportunity for a pharmacist who enjoys working as part of a multidisciplinary GP team and would like to develop their role within primary care. The successful candidate will have a varied role, supporting safe and effective prescribing, structured medication reviews, clinical monitoring and quality improvement.

You will work closely with our GPs, nursing team, pharmacy team and wider practice staff to improve patient care and support the delivery of practice and PCN priorities.

Main duties of the job
Main duties will include:

• Reviewing and managing medication, including structured medication reviews and medication optimisation.
• Processing and clinically reviewing DOCMAN correspondence, ensuring appropriate actions are taken and documented.
• Supporting delivery of PQS, QOF and other quality improvement requirements.
• Reviewing patients with high blood pressure, including medication optimisation and monitoring.
• Supporting cardiovascular disease prevention and long-term condition management.
• Monitoring medicines and ensuring appropriate blood tests and clinical monitoring are completed.
• Reviewing hospital discharge and outpatient correspondence where medication changes are required.
• Supporting safe, effective and cost-effective prescribing across the practice.
• Identifying prescribing issues, medicines safety concerns and opportunities for improvement.
• Working collaboratively with GPs, nurses, ACPs, paramedics and other members of the multidisciplinary team.
• Providing advice to clinicians and patients regarding medicines.
• Supporting audits, searches and practice improvement projects.
• Contributing to medicines-related policies, protocols and clinical audits.
• Helping the practice achieve its clinical, prescribing and quality targets.

Theale Medical Centre is a busy GP practice caring for approximately 11,000 patients. We have a friendly and supportive multidisciplinary team consisting of GPs, nurses, pharmacists, paramedics, ACPs and a wider administrative and reception team.

We use a number of digital systems to support our clinical work, including EMIS Web, Accurx and Ardens. Pharmacy Technician Job Description
Theale Medical Centre is looking for an enthusiastic, organised and motivated Pharmacy Technician to join our multidisciplinary practice team.

The Pharmacy Technician will play an important role in supporting the safe and effective use of medicines across our patient population. Working alongside our Clinical Pharmacists, GPs, nurses and wider multidisciplinary team, the role will support medicines optimisation, medication monitoring, clinical administration and quality improvement.

This is a varied role with opportunities to develop your skills and take responsibility for specific areas of medicines management within the practice.

Key Responsibilities
Medicines Optimisation

• Support the Clinical Pharmacists and wider clinical team with medicines optimisation.
• Undertake appropriate medication-related reviews and administrative medicines management tasks within scope of practice.
• Identify patients requiring medication reviews, monitoring or follow-up.
• Support safe, effective and cost-effective prescribing.
• Identify prescribing issues, medicines safety concerns and opportunities for improvement.
• Support patients with medication-related queries and signpost or escalate to the appropriate clinician when required.

DOCMAN and Clinical Correspondence

• Review and process relevant DOCMAN correspondence within agreed timescales.
• Identify medication changes, monitoring requirements and actions arising from hospital correspondence.
• Ensure appropriate actions are completed or allocated to the appropriate clinician.
• Update patient records accurately following review of correspondence.
• Escalate any clinical concerns or issues outside the scope of the role.

Medication Monitoring

• Support the practice with medication monitoring and recall processes.
• Identify patients requiring blood tests and other monitoring before medication can safely be prescribed.
• Support searches and recalls for medicines requiring regular monitoring.
• Help ensure monitoring requirements are followed up and appropriately documented.
• Work with the clinical team to reduce medication-related risks.

Blood Pressure and Long-Term Conditions

• Support clinical teams with patient searches and recalls relating to hypertension and other long-term conditions.
• Identify patients who may require medication reviews or monitoring.
• Support the practice with blood pressure-related prescribing and monitoring work within the scope of the role.
• Assist with cardiovascular disease prevention and medicines optimisation initiatives.

PQS, QOF and Quality Improvement

• Support the practice in delivering relevant PQS and QOF requirements.
• Assist with prescribing audits and quality improvement projects.
• Undertake searches and identify appropriate patient cohorts.
• Support medicines-related audits and improvement work.
• Assist with implementing changes to prescribing processes and practice protocols.
• Contribute to medicines safety initiatives and evidence required for CQC, PQS and other practice requirements.

Dispensary and Prescribing Support

• Work closely with the dispensary and clinical pharmacy team where appropriate.
• Support repeat prescribing processes and identify potential issues with repeat medication.
• Assist with medication reconciliation following hospital discharge and changes in care.
• Support the management of prescription queries and medicines-related administrative tasks.
• Promote safe and appropriate use of medicines.

Working as Part of the Multidisciplinary Team

Person Specification
Qualifications
Essential

• Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC).
• Holds an appropriate Pharmacy Technician qualification.
