Job summary
We are pleased to offer an opportunity for a Band 5 Occupational Therapist within our new Community Reablement Project.

The Social Care team are looking for an enthusiastic Occupational Therapist to join them in contributing to a caring and vibrant service working in the Baywide area.

The post holder will assist in the delivery of the Adult Social Care Reablement service Baywide.

The post holder will triage and assess new referrals and identify new reablement goals for service users following referral to reablement service. This ensures that service users' referrals, assessments and discharges from the service are managed effectively.

This post is also a supportive role to staff with advice and training relevant to the role (including support staff and referrers).

The Baywide Social Care Team are a small supportive team who enjoy mentoring team members and students and are passionate about delivering quality care to our community. We are a motivated and caring team to both the people we support and colleagues. The team is flexible and promotes a healthy work/home life balance.

Main duties of the job
  • The post holder is under the direct supervision of an Occupational Therapist Clinical Lead
  • There may be responsibility for the day-to-day organisation of and referral to support staff
  • The post may involve flexible hours, weekend and bank holiday working on a rota basis and provision of cover across the community and the community hospitals

Job responsibilities
Communication and working relationships

Ensure effective communication systems are maintained between Multi-disciplinary or Health and Social Care Team to provide consistent, well-co-ordinated patient/service user care

Relatives and carers to discuss patients condition and associated problems as appropriate to confidentiality

Evaluate care given and amend the occupational therapy process as necessary to ensure maximum effectiveness of treatment; utilise outcome measures appropriate to clinical / work area

Ensure that own professional knowledge is constantly updated in order to maintain a high standard of patient/client care; maintain a current CPD portfolio

Carry out holistic assessments and provide the multi-disciplinary team and family with recommendations regarding discharge from hospital or residential placement; to include appropriate level of support/care, adaptive equipment and minor adaptationsComply with the Unit Health and Safety Policy and procedures and ensure that a safe environment is maintained for patients, visitors and other staff

To ensure social care services / equipment are provided in line with the Care Act Criteria

Advice and support carers on aspects of care and the promotion of health to maintain care in the community for as long as possible

Participate in supervision and yearly review process in order to achieve personal development and a high standard of patient care.

Person Specification
Qualifications and training
Essential
  • Diploma/Degree in Occupational Therapy
  • Valid state registration with Health Professions Council
  • CPD portfolio and evidence of recent post-graduate learning relevant to area of clinical practice

Knowledge and experience
Essential
  • Evidence of good interpersonal skills with a wide range of people
  • Ability for clinical reasoning
  • Good written and verbal skills
  • Evidence of good problem-solving skills

Desirable
  • Confidence within the work setting
  • Experience of the use of outcome measures
  • Experience of health and /or social care IT systems

Specific skills
Essential
  • Pre or post-graduate experience working with patients / clients with following conditions
  • Orthopaedics; falls; Neurological conditions; cardiac and respiratory conditions
  • Pre or post-graduate experience of the following interventions
  • Home Assessment Visits; functional assessments; initial interviews

Desirable
  • Pre or post-graduate experience working with patients / clients in any of the following clinical/ work areas
  • Stroke rehab; social services; mental health; paediatrics; palliative care
  • Experience of specialist assessments such as cognitive, perceptual, wheelchair assessments, carer's assessments and Multi-Factorial Falls Assessments
  • Experience of following interventions: DFG process; cognitive or perceptual assessments; wheelchair assessments
  • Experience or skills for supervision of OT Assistant or Tech 3 and OT students
