Band 5 Staff Nurse - Healthcare for Older Persons
Closing date: 11 November 2026
Job summary
We are recruiting 3 Full Time Permanent Posts and 1 Full Time 12 month Fixed Term Post.
Ward 20A is a 25-bedded inpatient ward providing care for patients with complex co-morbidities, long-term conditions and complex discharge needs. We care for Trauma and Orthopaedic rehabilitation patients and those with conditions including Parkinson's disease, COPD and dementia. We work closely with a wide multidisciplinary team, including Dementia Outreach, Social Work and Palliative Care Specialists, to provide safe, patient-centred care.
Ward 20B is a 13-bedded inpatient medical ward, and we are excited to be opening a new 12-bedded Frailty Assessment Unit (FAU). We care for patients with complex co-morbidities, acute and long-term conditions, and complex discharge needs. The FAU will provide rapid assessment and management for older patients with complex health needs, supporting independence, preventing unnecessary admissions and promoting timely discharge.
We are looking for kind, compassionate and highly motivated individuals with a passion for delivering excellent patient-centred care. You will work as part of a supportive multidisciplinary team, act as a positive role model, maintain high standards of care, embrace new initiatives and continuously strive to achieve outstanding outcomes for our patients.
Main duties of the job
As a valued member of the nursing team, you will demonstrate a level of skill and understanding which enables you to undertake specific tasks delegated by the nurse in charge and in accordance with relevant procedures, guidelines and policies.
This critical position will work co-operatively as part of the multi-disciplinary team supporting colleagues in work associated with patient care. You will be encouraged to challenge and escalate poor practice/performance.
Use paper and electronic systems to retrieve and record patient information in line with Trust documentation standards and operational systems.
You will communicate clearly, effectively and appropriately with the multidisciplinary team, patients and their family, visitors and carers.
The wards both provide 24-hour care through a variety of shift patterns. The individual will work closely with the nursing team to carry out observations which include blood pressure, pulse rate, temperature, height and weight, fluid balance, oxygen saturations, blood sugar glucose monitoring. Observe and report to the registered nurse all changes in a patient's physical and psychological condition.
You will at times support families and carers following a patient bereavement or when bad news has been shared.
