An exciting opportunity has arisen for an experienced and motivated Heart Failure Specialist Nurse to join the Coventry and Rugby Heart Failure Team.

We are looking for a skilled clinician who is passionate about delivering high-quality, patient-centred care and improving outcomes for people living with heart failure. This role offers the opportunity to work across acute, community, virtual and home-based settings while developing advanced specialist knowledge and clinical leadership skills.

As a Heart Failure Specialist Nurse, you will provide expert assessment, management and support for patients with heart failure. Working autonomously and as part of a multidisciplinary team, you will deliver evidence-based care and contribute to the ongoing development of the service.

You will review patients through outpatient clinics, virtual consultations, telephone reviews and home visits, ensuring patients receive timely, personalised care throughout their heart failure journey.

You will be joining a friendly, supportive and well-established specialist team committed to delivering innovative heart failure services across Coventry and Rugby.

Main duties of the job
You will review patients through face-to-face and virtual clinics, Hospital at Home, home visits and telephone consultations.

You will work closely with the multidisciplinary team (MDT), GPs, healthcare professionals, patients and carers to ensure safe, effective and personalised care.

You will:

• Provide specialist assessment and management of patients with heart failure.
• Work autonomously across acute, community and virtual care settings.
• Lead and support the Heart Failure Nursing Team.
• Develop and implement evidence-based treatment plans.
• Support optimisation of heart failure medication in line with current guidelines.
• Refer patients for cardiac rehabilitation where appropriate.
• Consider and refer patients for cardiac device therapy when indicated.
• Promote patient education, self-management and improved outcomes.
• Liaise effectively with the MDT, GPs, healthcare professionals, patients and carers.
• Promote best practice in heart failure management in accordance with national and local guidelines.

Job responsibilities
This role requires the candidate to work across multiple sites, including University Hospitals Coventry and Warwickshire (UHCW), the Hospital of St. Cross, Coventry Community services, and the Hospital at Home programme. The post holder will provide expert clinical assessment and ongoing management of patients with heart failure. Lead and support the HF nursing team to deliver high-quality, patient-centred care across all service locations. Review patients in various settings, including face-to-face and virtual clinics, Hospital at Home, patients' homes, and via telephone consultations. Collaborate and liaise effectively with the multidisciplinary team (MDT), general practitioners (GPs), healthcare professionals (HCPs), patients, and carers. Recommend and implement evidence-based treatment plans, including optimisation of medications, referral to cardiac rehabilitation, and consideration of cardiac devices where appropriate.

Promote best practice in heart failure management in line with current national and local guidelines.

• NMC Registration
• Heart failure Module level 6/7 or equivalent
• Advanced Clinical Assessment Skills Level 6/7.
• Study at level six (degree/diploma)

• Non - Medical prescribing Level 6/7 or willing to undertake
• Advanced communication Course

Experience
Essential

• Extensive experience in heart failure
• Optimising HF medicines, referrals for cardiac devices and other therapies.
• Experience in HF Management
• Managing patient on H@H

• Leadership course
• Ability to run Nurse led Clinics.

• Extensive knowledge of heart failure and management
• Ability to present healthy lifestyles information to both patients as individuals and in groups.
