Job SummaryThe Neonatal Home Care Team at UHDB is recruiting dynamic, enthusiastic Band 5 Nurses to support a 7-day a week neonatal home-visiting service. The team helps safely transition babies from neonatal and transitional care units to home, supporting families with oxygen therapy, NGT feeding, short-term home phototherapy, and ongoing monitoring.
The role is specialist and community-focused, offering opportunities to develop advanced neonatal skills in a home environment while building strong, supportive relationships with families.
Main Duties of the Job- Support early discharge planning and coordinate structured home-care plans.
- Provide home-based neonatal care, including oxygen support, NGT feeding, and phototherapy.
- Work closely with the multidisciplinary team and the NACP-led Jaundice Clinic.
- Maintain high standards of care and accurate documentation.
- Empower parents, build confidence, and provide emotional and practical support.
- Manage an independent caseload safely and effectively.
- Ensure babies meeting criteria receive home-visiting support after discharge.
