Job summary
An exciting opportunity has arisen to join the Respiratory Inreach team at University hospitals Plymouth. This full time post is at band 6.

The successful candidate will work within a broad MDT and provide specialist advice and coordinate the care of inpatients with respiratory conditions with a particular focus on COPD and Asthma.

• To manage and co-ordinate care of inpatients with Respiratory conditions, with a focus on Chronic Obstructive Pulmonary disease and asthma.
• Work as part of Respiratory Inreach team, leading, teaching and delivering expert care.
• Act as an expert resource within the multi-disciplinary team.
• Contribute to resource management.

Main duties of the job

• Provide specialist patient focused advice and support to inpatients within case load.
• Support, ward staff and medical teams to identify and facilitate enhanced / supported discharge planning throughout the Trust.
• The post holder will have involvement in co-ordination and organisation of individual treatment programmes throughout the illness trajectory and work to develop communication across divisional and organisational boundaries.
• Provide Clinical supervision to other staff.
• Provide specialist advice, psychological support and education to patients, carers and health care professionals with regards to Respiratory care.
• Support the education and training of medical, nursing and allied health professionals as required.
• Carries out specialist respiratory recommendations and management plans.
• Works autonomously throughout the Trust supporting the Line manager in the development of the Inreach service according to need.
• Support audit and research projects and aid the development of policies and procedures as required.
• Contribute to resource management usingand allocating resources and equipment responsibly.
• May undertake research, clinical audits in own specialist area.

Job responsibilities
Clinical and Professional:

• As part of the Respiratory Inreach team manage caseload of patients in both the clinic, the Respiratory wards and across the Trust.
• Provide specialist advice on Respiratory conditions with a focus on Chronic obstructive Pulmonary Disease and asthma.
• Works as part of the Multidisciplinary team.
• Communicates highly sensitive, complex and contentious information to patients, relatives.
• Assess new and established patients with Respiratory conditions, identifying their psychological, physiological, emotional spiritual and cultural needs in relation to their disease and proposed treatment, providing a pathway of nursing care for the patient through liaison with other agencies, ie Long Term Condition nurses, GP, Community Respiratory Teams.
• Act as a patient advocate when relevant and be able to educate on all the treatment options to enable a patient to make an informed choice, negotiating with the medical team to agree a pathway of care.
• Provides continuity of care throughout the patient journey and liaising across organizations.
• Develop specialised programs of care and provide individualised advice including written and oral information to patients with Respiratory conditions, their relatives and careers.
• The Respiratory Inreach team acts as an expert resource within the Trust.
• Develop a progressive and dynamic approach to practice thus ensuring a responsive, quality up to date service.

Person Specification
Knowledge and Experience
Essential

• Demonstrable post registration experience.
• Demonstrable postgraduate experience working in a clinical area where the post-holder will have had frequent contact with respiratory patients.
• Demonstrable postgraduate experience in a broad variety of clinical settings.
• An awareness of current NHS and Health issues
• Good communication, organisational and time management skills

Desirable

• Demonstrable Counselling experience or qualification.

Qualifications
Essential

• NMC Registration
• Proven evidence of CPD

Desirable

• Degree in Nursing/Health studies
• Non-medical Prescriber
• COPD and Asthma course or willingness to do.
• Demonstrable experience

Aptitude & Abilities

• Advanced Respiratory skills.
• Advanced communication skills
• Able to work autonomously and as part of a team
• Demonstrable Experience supervising more junior staff
• Proven ability to manage own caseload
