Job summary
An exciting opportunity has arisen for the post of paramedic in general practice. The paramedic will work across both Weavers Medical in Kettering and its branch site in Mawsley to provide home visits for patients for same day conditions and/or review /exacerbations of existing conditions. The role will be fully supported and supervised by GPs at all times.

Weavers look after over 29,000 patients.

Main duties of the job
  • To provide home visits for patients across two practices Weavers Medical and Mawsley Branch for same days conditions and/or review/exacerbations of existing conditions.
  • To undertake all data recording ensuring all patient contacts are recorded in line with current protocols and guidance.
  • To work as part of an MDT to deliver care to patients.
  • To work under the clinical supervision of a GP at all times.

JOB DESCRIPTION
PARAMEDIC IN PRIMARY CARE
Title Paramedic in Primary Care
Band Band 6
Responsible to The Partners
Location Weavers Medical, Prospect House, Kettering
Hours per day 7.5 hours per day (Full time) 37.5 hours per week
10.00am 6.00pm Monday - Friday
Contract Type Permanent
Main Purpose
To provide home visits for patients of Weavers Medical, for same days conditions and/or review/exacerbations of existing conditions.
To undertake all data recording ensuring all patient contacts are recorded in line with current protocols and guidance.
To work as part of an MDT to deliver care to patients.
To work under the clinical supervision of a GP at all times.
Key Tasks
Same Day Home Visits
  • To undertake on the day home visits patients home or care home as triaged and directed by the on call GP.
  • To work autonomously within the community utilising enhanced clinical assessment and treatment skills to deliver patient interventions for acute medical conditions.
  • To undertake baseline observations for all patients visited.
  • To evaluate clinical information from examination and history taking and initiate appropriate treatment and/or referral.
  • To liaise with the on call GP and request advice and support as required.
  • To signpost appropriately to other services in the identified healthcare pathway.
  • To treat, as appropriate and within the scope of practice.
  • To arrange admission to hospital in line with current protocols and advice from the on call GP.
  • To advise and support patients on healthy lifestyles, health promotion and disease prevention.
  • To ensure all patient contacts are added to the patient record on the same day the visit is undertaken, providing a full and accurate record of decision making and using ARDEN templates for recording the data.

Review/Exacerbations of existing conditions
  • To undertake planned review visits for patients with existing conditions or exacerbation of current illness.
  • To assess and treat patients as advised by the on call GP and according to QOF and/or NICE guidelines.
  • To ensure all patient contacts are added to the patient record on the same day the visit is undertaken, providing a full and accurate record of decision making and using ARDEN templates for recording the data.

Support of Regular Vaccination Programmes
  • To assist the clinical team with regular, planned vaccination programmes for housebound patients. Vaccinations may include: flu, COVID vaccination boosters, Shingles and pneumococcal.

Other Tasks
  • Any other duties as considered relevant to the post.

Professional development/special requirements of the post
Must hold a relevant degree or diploma in Paramedicine or equivalent experience.
Must be registered with the Health and Care Professions Council HCPC.
Must have completed 2 year Consolidation of Learning period.
Must have a further 3 years post registration work experience at band 6 or higher.
Must have demonstrable experience of using diagnostic skills.
To have a full driving license.
To have access to a vehicle to be used for home visits.
Administration and Liaison
  • To ensure all communication media are checked during every duty period to facilitate good timely awareness and communication throughout the practice.
  • To report and write up any significant events or complaints and discuss with the practice manager and/or GP partner on duty.
  • To participate in clinical team meetings.
  • To participate Primary Health Care Team Meetings.
  • To maintain effective liaison with all other disciplines in the practice with relevant regard to confidentiality.

Health & safety
The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety policy, the practice Infection Control policy and published procedures. This will include
  • Using personal security systems within the workplace according to practice guidelines.
  • Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks.
  • Making effective use of training to update knowledge and skills.
  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and keeping them free from hazards.
  • Actively reporting health and safety hazards and infection hazards immediately when recognised.
  • Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holders role.
  • Undertaking periodic infection control training minimum annually.
  • Reporting potential risks identified.

Most instruction and communication of activity will be via the practice manager.

The post-holder will
  • Apply practice policies, standards and guidance.
  • Discuss with other members of the team how the policies, standards and guidelines will affect their own work.
  • Participate in audit where appropriate.

Person Specification
Qualifications
Essential
  • Must have acceptable GCSEs or higher including Grade C or above (or equivalent) in Maths and English.
  • Must be registered with the Health and Care Professions Council (HCPC).
  • Must hold a relevant degree or diploma in Paramedicine or equivalent experience.

Experience
Essential

Driving - Transport
