Job summaryThis is a new consultant post created to provide medical leadership to Hounslow's new Intensive Outreach & Rehabilitation Service, which brings together the existing Community Rehabilitation Service (CRS) and a new Intensive Outreach (IO) function under a single clinical and operational structure, each delivered as a distinct pathway with a single Team Manager. The post holder will hold overall medical responsibility for the Intensive Outreach pathway working closely with the Consultant responsible for the Community Rehabilitation Service (CRS) pathway (already in post).
This is a full-time post of 10 programmed activities, made up of two components. Six PAs (0.6 WTE) are allocated to the Intensive Outreach pathway described above. The remaining four PAs (0.4 WTE) are allocated to the Hounslow Early Intervention in Psychosis (EIP) service, where the post holder will provide consultant psychiatrist input to the EIP multidisciplinary team.
Main duties of the jobProvide medical leadership for the Intensive Outreach (IO) pathway, working in partnership with the Band 8a Team Manager.
Undertake senior assessment and direct clinical input for the most complex cases within the IO pathway, including risk and medication management, diagnostic assessment, Mental Health Act assessments and CTO reviews.
Support the team's duty function for urgent concerns and assessments.
Provide consultant psychiatrist input and clinical leadership to the Hounslow EIP multidisciplinary team, working in partnership with the EIP Team Manager.
Undertake senior assessments and direct clinical input for more complex EIP cases, including diagnostic assessment, risk and medication management, Mental Health Act assessments and CTO reviews, and hold senior medical responsibility for care delivered by the EIP team.
Provide early, easy-access advice to GPs and Primary Care Networks under a liaison and consultation model, contactable by telephone or email during working hours.
Facilitate smooth transitions into and out of the EIP service, including transition of young people from CAMHS at 18, and onward transfer to MINT or the IO and CRS pathways where appropriate.
