INFORMATION FOR APPLICANTS
Dear Applicant
Thank you for your interest in this post. Please read the following information before completing
your application form. The form is designed to ensure that you provide the necessary information
to determine how you meet the essential criteria. For your assistance, we have provided the
following useful tips on completing the application form. Remember it is important that you allow
sufficient time to submit your application form, ensuring that it is accurate and complete.
We require all sections of the application form to be completed in full. It is therefore important that
you complete each section of our application form giving the information requested. CV’s will be
accepted in support of your application form
Meeting the Criteria set out in the Personnel Specification
The Job Description describes the purpose, scope, main duties and responsibilities of the job. You
are advised to study the Job Description before completing your application form, then indicate on
your form how, in your opinion, your experience and/or qualifications are relevant to the post you
are applying for.
Please pay particular attention to the essential criteria detailed on the Personnel Specification and
make sure you address these requirements in full as shortlisting will be carried out on the basis of
the information contained on your application form. We cannot make any assumptions about such
information, and failure to address the requirements on the Personnel Specification will result in
you not being shortlisted.
You must provide specific and detailed information when answering the criteria questions on your
application form. Simply answering “yes” or “no” on your application form will not be sufficient and
failure to be specific will result in the rejection of your application form at a later stage in the process.
Identify relevant examples. This is very important as the examples you provide may be explored
further at interview and you may need to be prepared to talk about these in detail. It is your unique
role that the panel is interested in.
Clearly detail your personal involvement in any experience you quote; Write “I” statements e.g. I
managed the service, I managed a budget, I led on the initiative.
Do not use acronyms, complex technical details etc. Write for the reader who may not know what
the acronyms mean and include concise examples.

3
Please note that a selection of vacancies advertised within the Western Health and Social Care
Trust, are restricted to employees of the HSC. If this is the case, the Personnel Specification and
application form will include a note to advise that applicants must demonstrate that they are an
employee of the HSC at the closing date to be eligible. Applicants working for the HSC through a
recruitment agency at the closing date will also be eligible.
Completing Your Current / Previous Employment Details
When completing the details on your employment history, you must give exact dates of when you
started and finished each period of employment. These should be stated in the following format
DD/MM/YYYY. Please also include details of your reasons for leaving each post and explain any
gaps in your employment history. Provide a list of key duties that you have been responsible for in
current post / previous posts. Finally, if you are employed on a “Locum” or “As and When
Required” capacity – please specify the days/hours per week/month that you work.
Questionnaires
The questionnaire section of the application form asks you to detail how you meet each of the
essential and desirable criteria. Please ensure that you provide full and accurate information to
answer each of the questions. Simply answering “yes” or “no” on your application form will not be
sufficient and failure to be specific will result in the rejection of your application form at a later stage in
the process.
Please ensure you provide full details of all qualifications / professional registration required for the
post on the application form. Your relevant qualifications must be detailed in full.
Please note that some posts may require a full current driving licence and access to a car as part
of the essential criteria. It is important that you refer to both parts of this criteria when highlighting
how you meet this requirement, i.e. you must state that you have a full current driving licence, and
that you have access to a car. Failure to demonstrate how you meet both parts of this criterion will
result in you not being shortlisted.
If you are successful at interview, you will be required to produce original documentation of these
qualifications / professional registration prior to commencing the post.
References
You must provide the names and addresses of two referees. At least one of the named referees
must be your current or most recent employer. Please note that relatives must not be used. For

seek a reference from your last HSC / NHS employer (if applicable), even if this is not one of your
nominated referees.
Your references should cover previous employment / training / education within the past 3 years
i.e. needs to be current.

Statement of Policy Regarding Fitness to Practice Proceedings by a Licensing/Regulatory
Body and relating to criminal investigations in the UK or Overseas
Registration with the General Medical Council or General Dental council imposes on doctors and
dentists the duty to provide a good standard of medical care for, and behave appropriately towards
patients. Employers within the health and personal social services also have a duty to ensure that
patients receive a good standard of medical care and ensure as far as possible the safety of
patients. We therefore need to establish if you have been found guilty of a criminal offence, been
bound over or cautioned or are currently the subject of proceedings which might lead to a
conviction, an order binding you over or a caution, in the UK or any other country.
We also need to establish if you have been the subject of any fitness to practice proceedings in the
past, or if any fitness to practice proceedings are being contemplated, by a licensing or regulatory
body in the UK or another country and this is also reflected in the declaration.
This information will be treated in confidence and will not debar you from appointment unless the
selection panel considers that it renders you unsuitable for appointment. In reaching such a
decision we will consider the nature of the conviction/action, how long ago it took place and any
other factors which may be relevant.
Failure to disclose a criminal offence, having been bound over or cautioned or that you are
currently the subject to criminal proceedings which might lead to a conviction, an order binding you
over or a caution, or fitness to practice proceedings undertaken or being undertaken by an
appropriate licensing or regulatory body, may disqualify you from appointment, or result in
summary dismissal/disciplinary action and referral to the General Medical Council or General
Dental council for consideration if such a discrepancy came to light.
If you would like to discuss what effect any previous convictions, police investigations or fitness to
practice proceedings taken or being taken either in the UK or by an overseas licensing or
regulatory body might have on your application, you may contact the Human Resources
Department for advice.

5
Disability Requirements
We ask in the questionnaire section if you require reasonable adjustments, due to disability, to
enable you to attend the interview or undertake the duties of the post. Details of disability are only
used for this purpose and do not form any part of the selection process. If you require reasonable
adjustments to be made throughout the recruitment process please contact the Medical HR
Department to discuss.
Completing the Personal Declaration
It is important to remember that when signing the personal declaration section of the manual
application form or submitting your form via http://hscrecruit.com you are stating that the
information you have provided is true, accurate and complete. You are also advised that giving
wrong information or leaving information out could lead to the withdrawal of any offer of
employment, or dismissal if you take up a post.
Data Protection
The information you provide as part of the recruitment process will be handled in accordance with
the Data Protection Act 1998.
Completing the Equal Opportunity Monitoring Form
Please note that the information within the Equal Opportunities Monitoring section is regarded as
part of your application and you are strongly encouraged to complete this section. This information
is treated in the strictest confidence and is for monitoring /statistical purposes only. The selection
panel will not have access to this information at any stage of the recruitment process.
Submitting your completed form to the Recruitment & Selection Team
Applicants are encouraged to submit their applications online via www.hscrecruit.com and it must
be completed in full by the closing date and time. Applicants choosing to submit a manual
application form may do so and must ensure it is received by the Recruitment & Selection Team by
the closing date and time. Late applications will not be accepted. Manual application forms will
also not be accepted if they are incomplete or have been reformatted.
The standard Application Form is the only acceptable method of application. Information in
support of your application will not be accepted after the closing date and time for receipt of
applications.
Manual application forms should be posted to the relevant Medical HR Department:
Medical HR Department, Tyrone & Fermanagh Hospital, Omagh, Co Tyrone, BT79 0NS or
Medical HR Department, Altnagelvin Area Hospital, Londonderry, BT47 6SB
IF YOU ARE SUBMITTING A MANUAL APPLICATION, PLEASE ENSURE THAT YOUR
APPLICATION FORM IS RETURNED TO THE CORRECT ADDRESS AS DETAILED
ABOVE. APPLICATIONS TO THE WRONG ADDRESS WILL NOT BE REDIRECTED
OR RETURNED.

6
For hard copy applications using Royal Mail please note that 1
st
class mail does not guarantee next
day delivery. It is the responsibility of the applicant to ensure that sufficient postage has been paid
to submit the form to the correct address (as above) by the stated closing date and time. Existing
Health & Social Care staff should not rely on the internal postal system.
THE SELECTION PROCESS – WHAT TO EXPECT
Shortlisting
Once the vacancy closes, the shortlisting panel will consider each application and will make a
decision on whether or not the applicant meets the criteria outlined in the Personnel Specification.
Applicants relying on qualifications regarded as equivalent are responsible for providing
evidence as to how their qualifications meet the required standard. If equivalence is not
immediately clear to the shortlisting panel, no enquiries will be made by the Western Trust
and the candidate’s application may not proceed as a result.
Please note that the Western Trust reserves the right to use both the essential and the desirable
criteria at shortlisting. You should therefore make it clear on your application form whether or not
you meet the desirable criteria, as per the Personnel Specification. Failure to do so may result in
you not being shortlisted.
The Assessment Process
The assessment process for this recruitment exercise will be detailed on the Personnel
Specification. If you require any reasonable adjustments to allow you to participate you should
ensure this is clearly detailed in your application form.
Outcomes – once all applicants have completed the selection process, they will be contacted with
the results.
In accordance with best practice, all appointments within the Western Health & Social Care Trust
are made under the ‘Merit Principle’ where the best person for any given post is selected in fair and
open competition.
