Job summaryWe are seeking an experienced and motivated Practice Nurse to join our clinical team at Whitmore Reans Health Practice. This role offers an opportunity for a skilled nurse to work within a supportive general practice environment, delivering a broad range of nursing services to our registered patient population across our practices in Wolverhampton.
The successful candidate will work alongside GPs and the wider practice team to provide planned and preventative care, support patients with long-term conditions and contribute to improving health outcomes within our community.

The Practice Nurse will provide a range of clinical services to patients registered with Whitmore Reans Health Practice, working as part of the wider healthcare team to deliver safe and effective care. We welcome applications from nurses who are enthusiastic about general practice nursing and who are keen to contribute to a positive and forward-thinking team.
The post holder will undertake nursing assessments, develop care plans and support the management of patients with long-term conditions, including diabetes, respiratory disease, cardiovascular disease and hypertension. They will deliver preventative healthcare services including immunisations, cervical screening, NHS health checks and health promotion advice.

Job responsibilitiesClinical Responsibilities- Provide safe, effective, and patient-centred nursing care in a primary care setting
- Undertake long-term condition management clinics including (but not limited to) diabetes, asthma, COPD, hypertension, and cardiovascular disease
- Perform cervical screening in line with the NHS Cervical Screening Programe.
- Administer childhood and adult immunisations and vaccinations
- Carry out wound care, dressings, and minor injury management
- Support health promotion and disease prevention activities, including lifestyle advice and patient education
- Identify and escalate safeguarding concerns in line with practice policies
- Maintain accurate, contemporaneous clinical records using EMIS Web
Professional Responsibilities- Work within the NMC Code of Professional Conduct at all times
- Maintain up-to-date professional registration, revalidation, and CPD
- Participate in clinical supervision, appraisal, and personal development planning

Person SpecificationExperienceEssential- Registered Nurse (Adult) with current NMC registration
- Previous experience working in an NHS GP practice
- Sound knowledge of primary care nursing and NHS clinical pathways
- Perform cervical screening in line with the NHS Cervical Screening Programme
- Administer childhood and adult immunisations and vaccinations
Desirable- Experience working within a multidisciplinary or PCN team
- Experience contributing to clinical audits or quality improvement projects
- Experience using EMIS Web and AccuRx
QualificationsEssential- Registered Nurse (Adult) with current NMC registration
- Evidence of ongoing Continuing Professional Development (CPD)
- Previous experience supporting QOF, enhanced services, or local incentive schemes.
- Sound knowledge of primary care nursing and NHS clinical pathways
- Experience working in an NHS GP practice
Desirable- Experience working within a multidisciplinary or PCN team
