Job summary
We have a really exciting opportunity as part of a test of change for an experienced Occupational Therapist to join our dynamic, supportive, multi-disciplinary team at Bromyard Community Hospital.

Are you an innovative Occupational Therapist with a passion for improving rehabilitation outcomes and driving new ways of working?

We are seeking a motivated and forward-thinking Occupational Therapist to join our community hospital team to support the development and evaluation of therapy interventions for the Discharge to Assess pathways. This exciting role will focus on demonstrating the impact of earlier therapy involvement, reducing hospital-associated deconditioning, and delivering person-centred rehabilitation that enables patients to achieve their goals and return home sooner.

Working collaboratively within a multidisciplinary team, you will help shape and implement evidence-based approaches that maximise patient independence, improve functional outcomes, and support effective patient flow across the healthcare system.

We may consider a band 5 for this position dependent upon level of experience.

Main duties of the job
The Occupational Therapy Service in Community Hospitals aims to develop the patient's optimum level of function and independence in the physical, psychological and social aspects of life, facilitating timely, safe and sustainable discharge.

This is an exciting opportunity to contribute to service transformation and help shape the future delivery of rehabilitation within community hospitals. You will play a key role in demonstrating how earlier therapeutic intervention can improve patient outcomes, reduce deconditioning, enhance patient experience, and support timely discharge and patient flow.

Together, we are committed to delivering high-quality, person-centred rehabilitation that enables people to live as independently as possible and achieve the outcomes that matter most to them.

The focus of this role will be to plan and coordinate the safe discharge home for adults with a range of complex needs, liaising with community services to support a discharge to assess philosophy.

As a service we are proactive in supporting professional development and facilitating service improvement.

Education & Qualifications
Essential
  • Professional Qualification to degree level or equivalent in Occupational Therapy
  • HCPC Registered
  • Post Graduate specialist qualification/significant post graduate experience

Skills, Knowledge and Abilities
Essential
  • Broad and specialist knowledge of relevant patient conditions and therapy interventions
  • Demonstration of knowledge of core and specialist Therapy skills
  • Evidence of ongoing CPD in relevant area
  • Experience of multi-disciplinary communication, assessments and record keeping
  • Computer Skills including word processing and e-mail
  • Able to make independent, sound, clinical decisions and problem solve
  • Able to manage busy case load and prioritise
  • Ability to work autonomously using specialist knowledge, skills and initiative in sometimes complex situations acknowledging role boundaries

Experience
Essential
  • NHS or equivalent experience e.g. in inpatient areas, acute or community or community services
  • Experience of using/fitting adaptive equipment

  • Able to maintain confidentiality and professionalism within the environment that they are working
  • Able to demonstrate awareness of own limitations and need to ask for help
  • Evidence of ability to communicate effectively both verbally and in written documentation

  • Reliable

Other Factors
Essential
  • Ability to fulfil the travel requirements of the post
  • Able to meet physical & moving and handling requirements of post. I.e. re-positioning of a patient, use of moving & handling equipment, approved manual physical movement practices of patient
  • Able to deal sensitively and safely with blood and bodily fluids
