At Agincare, we know great care starts with great coordination. We're looking for a Care Coordinator at our Stourport-on-Severn who enjoys organising, problem-solving, and connecting the right carers with the clients who need them most.
The role of a Care Coordinator- Coordinate care services and client schedules
- Match carers with clients to meet individual needs
- Support the setup of new care packages
- Plan rotas to ensure continuity of care
- Communicate with families and healthcare professionals
- Record staff availability, absences, and emergency calls
- Participate in the on-call rota when required
- Willingness to cover the on-call phone (additional pay provided) Every Other Weekend
About you:We're looking for someone who is organised, people-focused and enjoys keeping things running smoothly, as well as:
- Strong organisational skills and attention to detail
- Ability to work confidently in a busy, fast-paced environment
- Full UK driving licence and access to your own vehicle
- Minimum of 12 months remaining on your right to work in the UK
