An exciting opportunity has arisen for a Band 5 Care Coordinator to join our welcoming and supportive South and West Community Mental Health Team.

Based in Bradford, we are seeking enthusiastic clinicians who are passionate about delivering high-quality, person-centred care to older adults within our diverse communities.

As a Care Coordinator, you will manage a caseload of service users, providing holistic assessments, care coordination, and recovery-focused interventions. You will support individuals with complex mental health needs, memory difficulties, reduced mobility, and social isolation, helping them maintain independence and improve their quality of life.

The role includes working within a multidisciplinary team and contributing to the ongoing development of the Memory Assessment Service, including the pre-diagnostic assessment pathway.

Main duties of the job
As a Band 5 Care Coordinator, you will deliver high-quality community mental health care to older adults, managing a caseload and coordinating person-centred support in line with Trust policies and professional standards.

You will:

• Undertake comprehensive mental health, risk and care needs assessments.
• Develop, implement and review care plans, acting as Care Coordinator for a designated caseload.
• Work collaboratively with service users, carers, multidisciplinary teams and partner agencies.
• Identify and manage risks, including deterioration in mental health, self-harm, suicide risk and safeguarding concerns.
• Coordinate referrals and access to appropriate services and interventions.
• Monitor, administer and support compliance with prescribed medication in line with Trust policies and professional guidelines.
• Maintain accurate, timely and comprehensive clinical records.
• Promote recovery, independence, equality, diversity and inclusion in all aspects of care.
• Work within the requirements of relevant legislation, including the Mental Health Act.

We are seeking motivated and compassionate clinicians with experience of working with people experiencing mental health difficulties. You will possess strong assessment, communication and care coordination skills, be able to work effectively within a multidisciplinary team, and demonstrate excellent organisational and time-management abilities.

Essential

• Ability to work without direct supervision.

Qualifications
Essential

• Registered Mental Nursing Certificate (RMN).

Desirable

• Evidence of post registration training in Mental health needs/ challenging behaviour

Experience
Essential

• Experience of working, or placement in a Community Mental health setting

Desirable

• Experience of being clinical adviser to junior staff or students.
• Experience in relevant area of Mental Health Nursing.
• Involvement in innovative nursing practice
• Worked as Named Nurse/Primary Nurse/Key worker/Team Leader.
