The ANP will be responsible for the delivery of evidence based nursing services, working as part of the practice multi-disciplinary team, delivering care within their scope of practice to our patient population.
We are looking for a Nurse Prescriber with experience of working in general practice. They will be responsible for supporting in a number of clinical areas such as health promotion, chronic disease management/QoF, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.
Main duties of the job- Deliver patient care via consultation either face to face or via telephone and referring patients to secondary/specialist care as required. This includes chronic condition reviews and safe effective prescribing of care plans and medications
- Develop, implement and evaluate individual treatment plans for acute and non-acute conditions
- Support the clinical team in the development of individual treatment plans for chronic disease patients
- Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
- Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
- Prescribe medication in accordance with PGD and within own scope of practice
- Review medication for effectiveness, need and national guidance
- Request and advice on prescribed or over-the-counter medication on side effects, common interactions and dosage
- Request pathology services as necessary, processing and interpreting results as required
- Maintain accurate clinical records in conjunction with extant legislation
- Ensure SNOMED codes are used effectively
- Maintain chronic disease registers
- Prioritise health issues and intervene appropriately
- Support the team in dealing with clinical emergencies
- Implement vaccination programmes for adults and children
- Support the clinical team with all safeguarding matters, in accordance with local and national policies.
About usAlderwood Medical Practice opened its doors to patients for the first time on February 16th 2015.
We have two GP partners, two part-time salaried GPs, a long-term locum GP, full time ANP, PN, HCA and Clinical Pharmacists. Supported by valued staff including management team, secretary and admin staff trained in workflow optimisation and coding.
Both partners are trainers and the practice hosts both medical students from Keele University and GP Registrars.
