Job summary
We are seeking an enthusiastic, experienced and compassionate mental health practitioner to join the Crisis Resolution and Home Treatment Team within Black Country Healthcare NHS Foundation Trust. The team provides timely, intensive, person-centred assessment and treatment for adults experiencing an acute mental health crisis, supporting people safely in their own homes wherever clinically appropriate and helping to prevent avoidable hospital admission or enable earlier discharge.

Working as part of a skilled multidisciplinary team, you will assess mental health presentation and dynamic risk, contribute to collaborative care and safety planning, deliver evidence-based interventions, and work closely with service users, carers and partner services. You will need sound clinical judgement, effective communication skills and the ability to respond calmly and flexibly in a fast-paced environment.

Main duties of the job

• Complete comprehensive mental health, risk, safeguarding and physical health assessments in community settings.
• Coordinate intensive home treatment, developing and reviewing person-centred care and safety plans with service users and carers.
• Use sound clinical judgement to manage changing presentation and risk, including crisis triage and admission gatekeeping.
• Deliver evidence-based interventions and support safe medicines management within professional scope and Trust policy.
• Work collaboratively with inpatient, community, primary care and partner services to prevent admission and support timely discharge.
• Provide clinical leadership and support to colleagues, maintain high-quality records, contribute to service improvement and participate in the 24-hour, seven-day rota.

Job responsibilities
As a Band 6 practitioner, you will work with a high degree of professional autonomy and accountability, taking a lead role in the assessment, formulation and delivery of care for people experiencing an acute mental health crisis.

Main duties include:

• Act as an autonomous practitioner, undertaking timely, comprehensive mental health assessments in the community and formulating presenting needs, mental state, capacity, physical health, safeguarding concerns and dynamic risk.
• Being an active participant in a shared caseload during episodes of intensive home treatment, developing, delivering and reviewing collaborative, recovery-focused care and safety plans with service users and, where appropriate, carers.
• Use advanced clinical judgement to deliver evidence-based interventions, monitor response to treatment and adapt plans promptly when presentation or risk changes.
• Undertake crisis triage and contribute to gatekeeping assessments for psychiatric admission, ensuring that viable alternatives are considered and that decisions are clearly documented, defensible and based on the least restrictive safe option.
• Make and communicate sound clinical decisions in complex or rapidly changing situations, escalating significant concerns appropriately and supporting coordinated responses with emergency and statutory services.
• Administer, monitor and promote the safe use of medicines, and undertake relevant physical health assessment and monitoring within professional scope, competence and Trust policy.
• Coordinate care across the multidisciplinary team, inpatient wards, community services, primary care, social care, emergency services and voluntary-sector partners to support admission avoidance, continuity of care and timely discharge.
• Maintain accurate and contemporaneous clinical records, provide clear handovers and reports, and contribute actively to multidisciplinary reviews, safeguarding processes and Mental Health Act pathways.
• Provide day-to-day clinical leadership, advice and role modelling to junior colleagues, students and support staff; contribute to induction, supervision, reflective practice and the development of a positive learning culture.
• Participate in implementing clinical audit action plans, being an agent for change in quality improvement and service development projects, applying learning to improve safety, experience and outcomes.
• Coordination of the shift as allocated duty worker, prioritising workload and supporting safe allocation of team resources.
• Participate in the 24-hour, seven-day rota, including evenings, nights, weekends and bank holidays, in line with service requirements.

Person Specification
Essential

• Education/Qualification: RMN, current registration, evidence of continued professional development
• Experience: managing acute mental illness, supporting clients with neuro-divergence, Case management, strong clinical leadership, audit processes. MDT working, supervision
• Knowledge: Conducting Biopsychosocial Assessment, Risk Assessment/ Formulation/ Management, Care planning, Safety planning, MHA/MCA/DolS, IP&C
• Skills/Personal Qualities: Interpersonal skills, Willingness for professional development, ability to manage emergency situations, self-motivated
• Trust Vision: be caring, enabling, working together, integrity
• A minimum of 2 years post qualification experience in a mental health setting
