We are seeking an enthusiastic, motivated and hard working nurse to join the colorectal cancer and stoma nursing team as a band 6 associate clinical nurse specialist. The right person for this job will have a genuine interest in the speciality, a desire to support patients, families and carers through treatment for colorectal conditions and strive to improve patient care and experience.

Main duties of the job

• Pre-operative counselling
• Counselling service for all colorectal, bowel disorder patients
• Encourage and advise pre-habilitation before surgery
• Pre-operative marking
• Management of stomas in the post-operative period
• Patient and carer education
• Partnership working with multidisciplinary team members
• Advisory service for bowel management, renal function, drug therapies
• Leadership in clinical care
• Prevention of health deterioration

• Nurse led follow up stoma clinics in the community

About us
The colorectal cancer and stoma nursing team support patients with colorectal conditions through a variety of treatments such as surgery, chemotherapy and radiotherapy, and with a focus on supporting those with both newly formed or problematic stomas. Support and care is provided through a variety of settings both in the hospital and the community and therefore access to a car is essential. There is an abundance of learning opportunities within this speciality, designed to ensure our patients receive the best possible care with highly qualified specialist nurses, and there is a wealth of experience within the team to ensure ongoing support and advice to any new team members.

Job responsibilities
Main Duties

• Support the Colorectal Cancer Clinical Nurse Specialists in meeting the care needs of patients.
• To develop and maintain high standards of practice.
• To be present in the outpatient department when patients are being informed of their diagnosis in order to provide support to both the patients and their families.
• To introduce self, role as key worker and provide contact details.
• To utilise holistic needs assessments and identify patients who might benefit from referral to other professionals (for example, a clinical psychologist, palliative care teams, social worker or benefits advisor)
• Assist medical staff in explaining about available treatment and acting as the patients advocate enabling them to make an informed choice.
• Support patients in adapting to changes in self-image, altered anatomy and function as appropriate.
• To be available for other professionals as a resource for expert advice.
• To work closely with groups such as patient support groups and to liaise as necessary with local colorectal teams and the Cancer Centres to ensure seamless care and facilitate open communication and co-ordination of care.
• To have highly developed communication and psychosocial skills, in order to recognise patients non-clinical needs as well as problems directly associated with their cancer or treatment.
• To develop into a clinical expert and ultimately into an autonomous practitioner.
• Support generalist staff by utilising knowledge, experience and understanding to make sound judgements in the care of colorectal cancer patients.
• There may be occasion when home visits for patients are required.
• To assist in the provision of stoma care.
• To offer support via telephone for post cancer treatment patients.
• To support the MDT in reviewing the cancer follow up pathway and utilising best practice in cancer survivorship.

Qualifications
Essential

• Registered Nurse
• Have or be working towards a teaching and assessing qualification
• Willingness to undertake a stoma care certificate
• Willingness to undertake further study in the specialist field

Desirable

• Advanced Communication skills
• Specialist colorectal cancer qualification
• Have or be working towards relevant degree

Experience & Knowledge
Essential

• Significant experience in a relevant clinical area at band 5 or above
