Part time - 22.5 hours

Are you a Band 5 Physiotherapist looking to develop your skills at a Band 6 level? Are you a Band 6 Physiotherapist who would like to develop your clinical skills in a different area?

If so, we have a brilliant opportunity to come and join us here in the District Therapy Team at Bolton Foundation Trust. We work across six neighbourhoods in Bolton to help provide a holistic community-centred provision.

We are seeking to appoint a dynamic, confident, innovative and flexible individual with excellent communication and organisational skills. The successful individual will need to engage closely with acute staff, community colleagues and social care staff to coordinate and provide high quality care in the community to avoid hospital admission and support hospital discharges.

Service is over 7 days 8am-6pm service. Staff currently work 1 in 6 weekends.

Main duties of the job
In order to successfully undertake this post you must be an experienced Physiotherapist looking to develop in this clinical area.

You will need some knowledge of community/intermediate care services.

You will be an effective team player, inspirational, flexible, adaptable, enthusiastic and fully committed to the development of patient focused, clinically effective and efficient services.

About us
The role will be part of a multi-disciplinary team consisting of Physiotherapists, Occupational Therapists, Therapy Instructors and Assistant Practitioners.

The service is part of the Neighbourhood model and has 6 bases across Bolton.

You will be a key player in providing assessment, personalised care planning and management of care to patients who may require intensive home support as an alternative to hospital care. The role will include participating in effective multi-disciplinary team meetings and integrated working with the Local Authority.

Job responsibilities

The service is currently operating over 7 days between 8am and 6pm.

Qualifications
Essential
  • Degree or equivalent in Physiotherapy
  • Current professional registration as a Physiotherapist

Experience
Essential
  • Treatment of a broad range of conditions relating to clinical speciality
  • Evidence of leading and implementing clinical change
  • Supervision and/or mentoring of staff/students.
  • Multi-disciplinary/multi-agency collaborative working.

Desirable
  • Substantial NHS experience

Skills
Essential
  • Ability to communicate highly complex pathologies and diagnostic reasoning to all levels in the medical and non-medical field at the appropriate level.
  • Able to undertake assessment in difficult complex situations
  • Sound knowledge of psycho-social problems and ability to gain patient confidence and co-operation.
  • Able to work independently as lead specialist, demonstrating good leadership skills.

Knowledge
Essential
  • Understanding of clinical governance framework and implications for practice including audit and research governance.
  • Understanding of current NHS/government policy/Public Health/NICE guidelines on organisational change and the ability to adapt these appropriately to local service needs through publication of documents.
  • Change management skills

Other
Essential
  • Ability to work flexibly and travel across the service footprint to meet business needs
  • Able to work under pressure and constantly respond and adapt to frequent changes in circumstances.
