Job summary
Applications are invited for a Part-Time (8PA) post of Consultant Geriatrician at Bradford Teaching Hospitals NHS Foundation Trust.

The successful candidate will work alongside an enthusiastic team of 14 Consultant Geriatricians, matrons, nurses, ANPs, supported by speciality registrars and FY1/2 doctors.

The elderly care department includes 4 acute wards, 4 community hospitals and a virtual ward. Intermediate care services include Community Hospitals and Virtual Ward, providing opportunities to develop a more effective interface between acute and community-based services for our older population with a discharge-to-access model.

The post holder will work with the existing team of consultants to deliver the Elderly Care service across the trust, with particular focus on Interface geriatrics and developing community-based services with a focus on admission and readmission avoidance. The post holder will be expected to undertake a weekly Urogynaecology clinic

The post holder will be involved in the expansion of orthogeriatric services at the trust to include frail older people based on the orthopaedic ward with fractures other than hip fractures and involvement of geriatricians in the elective orthopaedic waiting list.

Main duties of the job
The post holder will be expected to support the Virtual Ward, which has been established as an enabling multidisciplinary team (nursing, medical, therapy and social care) to support older people at home. The team provides a supported discharge to patients from acute wards via a "step-down" model of discharge to an assessment or an ambulatory care pathway. The virtual ward also supports admission avoidance by providing the same multidisciplinary team to patients "stepped up" from community or accident and emergency department. Moving forward, we are working towards embedding a 'comprehensive geriatric assessment' in all patients, with a view to preventing admissions from primary care.

The post holder will develop services aimed at avoiding readmissions to hospital, through active follow-up of high-risk patients on discharge from hospital to avoid repeated acute admissions where possible.

In addition, the post holder would be expected to take on significant responsibility in the supporting activities for the department within areas of clinical governance, taking into account any areas of interest and consultant colleagues' current commitments. This includes participation in the department's service development, clinical governance, MDT review, clinical trials, mortality and business meetings.

Job responsibilities
The postholder will work alongside the existing consultant team to deliver the Elderly Care service across the Trust, with a particular focus on interface geriatrics and the development of community-based services that support admission and readmission avoidance.

The postholder will undertake regular inpatient ward rounds and provide medical support to the Elderly Assessment Unit, with a primary focus on avoiding unnecessary admissions and enabling patients to receive appropriate care within the community.

They will also support the established Virtual Ward, an integrated multidisciplinary service involving nursing, medical, therapy and social care colleagues. The team supports older people at home through:
  • A step-down model, facilitating supported discharge from acute wards and discharge-to-assess pathways.
  • A step-up model, preventing avoidable admissions by supporting patients referred from the community or the Emergency Department.
  • Ambulatory care pathways where appropriate.

The postholder will undertake a weekly outpatient clinic.

They will contribute to the expansion of the Trusts orthogeriatric service. This will include supporting frail older patients admitted to orthopaedic wards with fractures other than hip fractures and providing geriatrician input into the elective orthopaedic waiting list.

The postholder will also help develop services aimed at reducing hospital readmissions through the proactive follow-up of high-risk patients following discharge, supporting them to remain safely within the community wherever possible.

In addition, the postholder will be expected to undertake significant supporting professional activities within the department, including responsibilities relating to clinical governance. These responsibilities will take account of the successful candidates areas of interest and the existing commitments of consultant colleagues. They will participate in service development, clinical governance activities, multidisciplinary reviews, clinical trials, mortality meetings and departmental business meetings.

Experience
Essential
  • Evidence of completing an approved period of training in Geriatric Medicine with Continence capability in practice (CiP) or equivalent experience.

Qualifications
Essential
  • MBChB or equivalent, MRCP or FRCS of equivalent to specialty (UK) or equivalent, Member of the Royal College of Physicians (UK) or equivalent, CCT or within 6 months of AAC date, On Specialist Register with GMC and continue to hold a licence to practice
