Job summary
The post holder will be a nurse with vascular surgery, hot clinic, diabetic foot, outpatient and wound care experience working within their professional scope of practice as part of the multidisciplinary team, delivering high-quality care to patients with peripheral arterial disease, including abdominal aortic aneurysm (AAA), critical limb ischaemia and transient ischaemic attack (TIA), as well as those with superficial venous disease, including ulceration, bleeding and superficial venous thrombosis.

They will demonstrate safe, evidence-based clinical practice, utilising advanced assessment, diagnostic and clinical decision-making skills to manage patients presenting with both acute and chronic vascular conditions.

The post holder will work effectively across professional and organisational boundaries, promoting collaborative interdisciplinary working to enhance patient outcomes, improve patient experience and ensure the efficient use of healthcare resources.

This flexible Clinical Nurse Specialist (CNS) role will involve close collaboration with Vascular Consultants, Vascular Clinical Nurse Specialists, Vascular Scientists and other members of the multidisciplinary team. The successful candidate will be supported in developing towards autonomous clinical practice, enabling them to work independently and contribute to the delivery of high-quality, patient-centred care of vascular patients across the network.

Main duties of the job

• Maintain specialist knowledge of peripheral arterial disease, diabetic foot disease, venous disease, AAA and leg ulcer/complex wound management.
• Apply advanced vascular assessment skills including ABPI, pulse assessment and wound assessment.
• Maintain knowledge of vascular investigations, treatment pathways and surgical interventions.
• Work in accordance with NICE guidance, national standards and local vascular pathways.
• Make autonomous decisions regarding investigations, referrals and follow-up care.
• Use clinical skills to inform independent decision making and management plan.
• Coordinate patient pathways across the Bristol, Bath and Weston Vascular Network.
• Manage and prioritise a complex outpatient caseload.
• Organise rapid-access appointments, investigations and MDT reviews.
• Undertake comprehensive vascular assessments, including ABPI and complex wound assessment.
• Perform clinical history taking and physical examination.
• Interpret and accurately document clinical findings and investigations.
• Provide specialist assessment and management for patients with peripheral arterial disease, venous disease and diabetic foot complications.
• Undertake autonomous clinics for new and follow-up patients.
• Educate patients and families regarding diagnosis, treatment options and risk reduction.
• Coordinate investigations, referrals and ongoing care.

Qualifications
Essential

• RGN with current NMC registration
• Ability to make decisions, act and work independently
• Hot clinic vascular experience
• Ability to confidently undertake ABPI, TBI and TCp02
• Experience of compression bandaging and independent leg ulcer management
• Full Clean driving license
