Job summary
An exciting opportunity has arisen for a Band 7 Clinical Nurse Specialist to join our Dementia Outreach Team and become part of a service that really makes a difference to patients experiencing a dementia-related crisis, and their families.

As an experienced and motivated Registered Mental Health Nurse you will be responsible for providing clinical and managerial leadership with mentoring and educational input to the Dementia Outreach Team multidisciplinary team. This includes acting as a clinical leader and expert practitioner liaising, guiding and advising the multidisciplinary team in the provision of optimum patient experience for people accessing support and assessment. They will also be responsible for supporting the effective role modelling, ongoing management and clinical supervision, and mentorship to the team.

You will ideally have experience of working with people with dementia. The ability to work in partnership with carers or family members and services in the community is essential. Working closely with nursing, medical and allied health professionals, you will provide clinical leadership, contribute to service development, support education and training, and promote best practice in mental health care across the Trust.

Main duties of the job
The post holder will be responsible for managing their team by providing mentoring, supervision and education in relation to all managerial, professional and performance processes. Within the leadership role they will be responsible for supporting the team and providing feedback on the evaluation of good and poor practice, identifying training and education opportunities to improve practice and delivery.

The post holder will be expected to work in line with the Trust values and behave in a manner that facilitates the development of autonomous teams and individuals that are empowered, engaged and committed to continually delivering excellent care with compassion. The post holder will be an advocate for supporting dementia care.

They will also be expected to play a proactive role in quality and service improvement and working closely with the Service Manager and the multidisciplinary team, leading auditing of clinical standards of care. Resourcing and supporting on-going training, education and support to the multidisciplinary team in relation to mental health and learning disabilities to ensure that people accessing the service receive equitable support and care whilst improving patient outcomes.

Qualifications
Essential

• Registered Nurse (Mental Health) with live NMC registration
• Supplementary or Independent non-medical prescribing qualification (or commitment to undertake this course)
• Nurse Mentorship/Practice Assessor Qualification or equivalent qualification to supervise nursing students
• Current, clean driving licence
• Evidence of on-going professional development and post-registration training
• Evidence of training in risk assessment and risk management

Desirable

• Training in providing clinical supervision

Experience
Essential

• Effective time management and prioritisation skills
• Experience in the assessment and management of people with complex mental health conditions including serious mental illness
• Ability to work effectively in urgent/crisis situations in order to maintain the safety of both staff and clients
• Ability to communicate effectively, both verbally and in writing, with service users and colleagues across multi-disciplinary groups
• Ability to demonstrate a creative and flexible approach in order to deliver a high standard of care
• Ability to provide clinical supervision to colleagues
• Knowledge of and ability to work as part of a multi-disciplinary team
• Ability to assess, plan, implement and evaluate care plans
• Ability to complete the training in the management of violence and aggression/breakaway
• Evidence of undertaking clinical audits and effectively using the data from these audits for quality improvement/service development purposes
