Main duties of the job
Working in a well established multi-disciplinary team, You will be an experienced and registered Nurse, Paramedic or Physiotherapist with the knowledge, skills and ability to practice autonomously without direct supervision. You will utilise advanced clinical skills to assess, diagnose, treat and refer patients requiring clinical services and recognise the need for and initiate referral for relevant diagnostic investigations or for advice of relevant specialities. This will be via telephone triage and consultation and carrying out Home Visits as required.

You will have excellent communication skills (verbal and written) when dealing with patients and carers and have the ability to make clear decisions with confidence and communicate these effectively. You will have excellent Team skills and be able to demonstrate experience of multi-disciplinary team working.

About us
Burnley Group Practice is a 9 Partner, busy training practice set in the heart of Burnley and serving approximately a quarter of the population its population. We are based over three sites across Burnley and candidates will be required to work from each site on occasion.

We have a well established multi-disciplinary team with a wide range of clinical skills and who are supported by an established and experienced Reception and Admin Team.

We are a hard working Practice which believes in the delivery of good quality patient care but also believe in maintaining a good home/work balance.

Job responsibilities
The post holder will be responsible for the autonomous clinical assessment, treatment and/or referral or giving advice as appropriate to both adult and children across a wide spectrum of clinical conditions.

The post-holder will

• Provide the choice of direct access to a NP, over the telephone, and by appointment, for the general practice population.
• Make professionally autonomous decisions for which he/she is accountable
• Provide a first point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan
• Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports
• As an independent prescriber - to prescribe safe, effective and appropriate medication as defined by current legislative framework
• Provide safe, evidence-based, cost-effective, individualised patient care
• Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care
• Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills

Qualifications
Essential

• Relevant specialist registration e.g. NMC or HCPC
• Masters Degree in Advanced Clinical Practice
• Minor Illness qualification at Degree level assessed by OSCEs
• Independent non-medical prescriber

• Paediatric Nurse/Paediatric ANP
• Teaching/mentoring qualification

Experience
Essential

• Post registration experience
• Experience working autonomously in a complex clinical setting
• Experience of evaluating safety and effectiveness of own clinical practice

• Two years' experience post MSc qualification
• Interpreting and implementing local and national policy agendas for health
