Clinical Case Manager MidlandsBush & Company Rehabilitation is seeking a Clinical Case Manager to join our team in the Midlands. This is a part-time, temporary position offering a competitive salary.
Role OverviewAs a Clinical Case Manager, you will be responsible for coordinating and managing rehabilitation programmes for clients with complex needs. You will work closely with clients, their families, and a multidisciplinary team to ensure the highest standard of care and support.
Key Responsibilities- Assess clients' needs and develop individualised care plans
- Coordinate with healthcare professionals and other stakeholders
- Monitor progress and adjust plans as necessary
- Provide support and guidance to clients and their families
- Maintain accurate and up-to-date records
Requirements- Registered healthcare professional (e.g., nurse, physiotherapist, occupational therapist)
- Experience in case management or rehabilitation
- Excellent communication and organisational skills
- Ability to work independently and as part of a team
- Full UK driving licence and access to a vehicle
