Clinical Case Manager - Midlands

About the Role
We are looking for a dedicated Clinical Case Manager to join our team, covering the Midlands region. You will be responsible for managing complex rehabilitation cases for clients with life-changing injuries.

Key Responsibilities

• Manage a caseload of clients with complex rehabilitation needs
• Conduct comprehensive clinical assessments
• Develop and implement person-centred care plans
• Coordinate with multidisciplinary teams, therapists, and families
• Monitor progress and adjust care plans as required
• Prepare detailed reports and documentation

About You
You will be a qualified healthcare professional with experience in case management and rehabilitation. You will have strong assessment and communication skills.
