Job Summary
We are looking for a motivated and compassionate GP to join our team in rural Wales. This is an excellent opportunity to work alongside experienced clinical and administration colleagues.

Working as part of the multidisciplinary team you will play a key role in delivering high quality healthcare for patients who can face challenges accessing services due to geography and transport limitations.

Main Duties of the Job
  • Provide high quality patient centred care through 15 minute face to face appointments, telephone appointments and digital consultations. (Accurx)
  • Assess diagnose and manage a wide range of acute and chronic medical conditions.
  • Prescribe medication safely and effectively in line with current clinical guidelines.
  • Use of Electronic Prescribing
  • Undertake home visits where clinically appropriate.
  • Work collaboratively with the multidisciplinary team.
  • Maintain accurate and timely clinical records.
  • Mentor or support medical students from Cardiff University.
  • Support local Community hospital and colleagues with weekly ward rounds and daily visits.
  • Promote and support QI project.
  • Attend clinical meetings.

About Us
Based in rural Wales we are a friendly team with a supportive and inclusive working environment. We have 4800 patients registered our team consists of 3 GPs (one is retiring), 3 Advance Nurse Practitioners. 2 Practice nurses, 1 Band 4 Nurse Assistant Practitioner and 2 HCA assistants also administration and clerical colleagues.

We have daily Advanced Nurse Practitioner triage, chronic disease management clinics, Spirometry clinics, with a significant commitment to the local Community Clinic, we enjoy and encourage undergraduate teaching and we are a high QI achiever.

Job Responsibilities
Job title: Salaried or Partner General Practitioner

Hours: 4-8 sessions

Job Summary:
The post-holder will work as part of a team, dealing with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered patients.

Clinical Responsibilities:
  • Conduct surgery consultations, telephone consultations, home visits and manage admin results, prescriptions, correspondence.
  • Making professional, autonomous clinical decisions in relation to presenting problems.
  • Assess, diagnose and manage a broad range of conditions
  • Screening patients for disease risk factors and early signs of illness.
  • Referring to other care providers as appropriate.
  • Recording clear and accurate consultation notes to agreed standards.
  • Support audits and quality improvement.
  • The post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

Other Responsibilities within the Organisation:
  • Follow all practice policies (prescribing, confidentiality, data protection, health & safety
  • Commit to ongoing learning and evidence-based practice.
  • Participate in audits, clinical standards, and training events.
  • Contribute to the training of GP trainees and medical students

Confidentiality:
Maintain strict confidentiality regarding patient, staff, and organisational information. Share information only with authorised individuals in line with policy.

Qualifications - Essential
  • Fully Qualified GP
  • Full GMC Registration
  • General Practitioner Certificate of Completion of Training CCT
  • Eligibility to practice in the UK independently
