Job SummaryWe are pleased to be able to advertise for a community mental health Nurse/Social Worker/Occupational Therapist. We are seeking a mental health professional who is looking to be at the forefront of future healthcare and integrated working within the Specialist Mental Health Services for Adults. As a multi-disciplinary team we provide holistic assessment and treatment pathways for people with mental health needs. Our team is made up of Clinical Psychologists, Consultant Psychiatrists, Occupational Therapists, Social Workers, as well as Mental Health Nurses. We also provide regular CPD sessions, reflective practice, staff support sessions, individual and group supervisions, and academic training, as well as access to external training resources.
Main duties of the job- To assess service users' mental and physical health status and needs.
- To assess other needs around their health and wellbeing.
- To undertake and input into risk assessments jointly with the MDT and develop robust risk management plans, according to Trust policy.
- To ensure development of robust care plans and delivery of those in partnership with multi-disciplinary colleagues and a range of health and social care agencies, within the DIALOG+ framework.
- To identify where best those needs can be met and facilitate signposting, onward referral or rapid access to relevant professionals and services and to provide support/advice to the referring GP on effective management.

The post will ensure the mental health needs of adults with mental illness referred to Hillingdon CMHHs are appropriately assessed, supported and a high level of quality care is delivered within available resources.
