Job Summary
This post offers an excellent opportunity to join our growing Hospital at Home (H@H) service as a Band 6 Nurse / Paramedic, working as part of a skilled and supportive multidisciplinary team delivering hospital-level care to patients within their own homes. Operating as an integral part of the Urgent Community Response (UCR) pathway, the post holder will contribute to the safe management of patients who would otherwise require an acute hospital admission, supporting both admission avoidance and early supported discharge for the borough of Hillingdon.

Main Duties of the Job
The post holder will carry out comprehensive clinical assessments, deliver evidence-based interventions, and actively contribute to daily MDT reviews for patients admitted to the Hospital at Home pathway. You will manage a defined caseload of patients with complex and acute needs, including those with frailty, and will be expected to make confident clinical decisions, escalating appropriately to senior clinicians and the Senior Clinical Decision Team where required. In keeping with CNWL's SCARF framework, you will demonstrate safe, accountable and reflective practice, adapting to the varied and fast-paced nature of community-based acute care to ensure patients receive timely, effective treatment in the right place at the right time.

The role requires you to develop and maintain a broad range of clinical competencies including independent assessment in the home environment, use of digital monitoring tools, point of care testing, cannulation, IV therapy administration, catheter care, and frailty management. You will work in close partnership with GPs, acute hospital teams, pharmacy, social care and community nursing living CNWL's value of partnership to ensure patients experience a seamless episode of care.

Job Responsibilities
  • To assess, plan, implement and provide evaluation of health care needs in conjunction with patients, their carers and other appropriate members of the multi professional team.
  • To have agreed areas of responsibility and work as part of the team.
  • To provide nursing care to patient in the community
  • To see patients with potentially undifferentiated diagnosis including; objective assessment, physical assessment, treatment, follow-up and developing care plans.
  • To ensure all patients on the caseload are managed well and discharged safely either to their GP or for further ongoing assessment, rehabilitation treatment and care.
  • To be responsible for keeping up to date with current issues and events in Primary and Secondary Health Care
  • Liaising with community services partners
  • To be an active member of the team and the teams development.
  • To provide support and health education for patients and carers through effective interpersonal skills.
  • To comply with professional codes of conduct.
  • To be able to work flexibly and relieve colleagues/rotate across the three boroughs as required and requested by the line manager to meet the needs of the service.
  • To ensure that own mandatory training is kept up to date in line with Trust policies.
  • Post holder will undertake delegated line management of staff, both informal and formal basis

Person Specification
Essential to Role
  • Registered Nurse Level 1 with current registration with the NMC
  • Experience in Community

Desirable
  • Substantial post registration experience (Preferably in Paramedic care, UCC, ITU, AMU, MAU, CCU or A&E)

Clinical
Essential
  • Evidence of acute disease management and crisis care
  • Evidence of ability to direct and coordinate programmes of care working autonomously and collaboratively

Desirable
  • Evidence of ability to communicate both verbally and in writing at all levels in a multidisciplinary forum and with patients and carers
