CNWL are currently looking for a motivated full-time Speciality Doctor within the Addictions team at ARCH, Hillingdon.

Main duties of the job
Clinical duties will be delegated and supervised by the Consultant Psychiatrist. Wednesday afternoons are allocated to Continuing Professional Development (CPD) activities, coinciding with the academic programme at our local mental health unit at the Riverside Centre, but also quality improvement, research and training.

The post is designed to be flexible and the post holder will be expected to respond to strategic developments and the needs of the service. The post holder will work closely with other doctors across CNWL in Hillingdon. The post is non-resident.

CNWL's Addiction Directorate is a large, well-established, multidisciplinary NHS provider offering a wide range of high-quality addictions services to the diverse and multicultural population of London, Milton Keyes and the East of England. In addition to borough-based addiction treatment services, we also provide a club drug clinic, behavioural addictions (gambling and gaming),hospital liaison, homeless outreach and university in reach services.

Addictions Recovery Community Hillingdon (ARCH) is a partnership model service led by CNWL, together with a number of voluntary sector services (namely v-i-a and BoB). The service provides a comprehensive range of addiction treatments to Hillingdon residents with drug and alcohol problems and their carers.

ARCH is based at Old Bank House in Uxbridge town centre, providing easy access for Hillingdon residents.ARCH comprises a large multi-disciplinary team that includes 1 Consultant Psychiatrist, 1 Specialty Doctor, 1 Specialist Trainee (ST4-6), 1 Core Trainee (CT2-3), 1 Foundation Year trainee (FY2), a physician associate, nurses, a psychologist, social workers, recovery practitioners, outreach workers and peer support workers.

Job responsibilities
Clinical Duties

• To work as a member of the multidisciplinary team and participate in patient assessment and the delivery and evaluation of treatment. Responsibilities include assessment of new patients, review of existing patients and planning treatment with keyworkers.
• To be responsible for day-to-day medical assessment and decision-making (with appropriate support and working within limits of competence).
• To liaise closely with patients keyworkers, who will be primarily responsible for co-ordinating their care. The post holder may be required to hold a small caseload, although the post holder will primarily review existing patients collaboratively with their allocated keyworker. Follow-up reviews may be facilitated should the patients presentation change or require a care plan review.The model of addictions services is different to community mental health outpatient clinics - referrals are from keyworkers or duty workers in the service and are jointly facilitated to support further assessment or management. The number of assessments per week varies depending on patient complexity, need or if patient is new or follow-up.
• To provide clinical advice to team members on a 1:1 basis and within MDT meetings.
• To manage community patients undergoing detoxification for drug and/or alcohol dependence.
• To initiate, stabilise and maintain patients on opiate substitute treatment (OST).
• To undertake mental health reviews on patients.
• To provide health care appropriate to the patients physical needs and make appropriate referrals to medical colleagues.
• To provide support to designated and prospective Non-Medical Prescribers.
• To provide formal / informal training to staff locally as well as any students (medical, nursing and social work) attending the service.
• To liaise closely and communicate effectively with colleagues in statutory and non-statutory services concerning the treatment and management of our patients. This includes colleagues in settings such as primary care, the Acute Trust, mental health services, Adult Social Care and the Probation Service.
• To regularly sign prescriptions, usually generated in batches and for controlled drug medications, adhering to CNWLs Medicines Policy.
• To ensure that patients are assigned ICD-10 (will change to ICD-11) diagnostic coding and that these are properly recorded on the electronic patient record.
• To contribute to the completion of serious incident reports as required.
• To contribute to the completion of coroners reports as required.
• To support the service in achieving key performance indicators in accordance with Public Health Englands National Drug Treatment Monitoring Service (NDTMS).
• To maintain comprehensive contemporaneous documentation on the Trusts electronic patient record (SystmOne).

General Duties

• To deputise for absent colleagues when required.
• To supervise junior medical staff
• To fulfil the Trusts mandatory and statutory training requirements.
• To adhere to the Trust's policies and guidelines.
• To support creating and revising clinical protocols and service information leaflets.
• To participate in regular training events within the service and within the Trust.
• To participate / lead projects in research, quality improvement, training sessions and clinical meetings.
• To engage proactively in local care quality and performance management initiatives, as appropriate.
• The post holder may be required to undertake additional duties in the event of occasional emergencies and unforeseen circumstances.
• The post holder may be required to work at other sites within the Trust in order to meet the needs of the Trusts services. Any significant changes to the job role will be subject to consultation.
• There is no requirement for the post holder to undertake Mental Health Act assessments or be approved under Section 12(2) of the Mental Health Act.

Person Specification
Education and Qualifications
Essential

• Full GMC registration and current licence to practice

Desirable

• Has obtained MRCPsych (or equivalent) qualification, or 1 or more of its constituent exams.

Previous Experience
Essential

• Completed at least 4-years postgraduate training, at least 2 of which are in a specialty training programme (or equivalent portfolio pathway)
• Psychiatry training / experience and competencies

Desirable

• Experience of working within a variety of mental health inpatient and community settings
• Experience of working in an addictions treatment setting
• Experience of prescribing opiate substitute treatments
• Completed a quality improvement project
• Significant involvement in research

• Able to treat service users with respect and dignity at all times, adopting a culturally sensitive approach that considers the needs of the whole person
• Able to build constructive relationships with warmth and empathy, using good communication skills
