This role offers a unique opportunity to influence the future direction of Continuing Healthcare services, contribute to system-wide transformation programmes and support the delivery of integrated care across North Central London.

Main duties of the job

  • Manage service performance, workforce planning and operational delivery to ensure key targets and quality standards are achieved.
  • Contribute to the development and implementation of service improvement initiatives, pathways and transformation programmes.
  • Work collaboratively with system partners to improve patient flow, discharge pathways and integrated care delivery.
  • Support budget management and ensure care packages deliver high-quality outcomes and value for money.
  • Lead on governance, quality assurance, risk management and compliance with relevant statutory and regulatory frameworks.
  • Represent the service at senior-level meetings and contribute to strategic planning across the wider health and care system.
  • Promote equitable, patient-centred services that improve outcomes and reduce health inequalities for patients, families and carers.
