We are seeking an experienced and motivated Advanced Clinical Practitioner (ACP) for 20-37.5 hours per week, to deliver frailty and complex care services within our Integrated Complex Care Team (ICCT) as part of our Single Neighbourhood Health (SNH) Service.
You will play a pivotal role in improving outcomes for patients with frailty, multimorbidity and complex needs, supporting proactive care, reducing avoidable hospital admissions, and leading multidisciplinary team (MDT) working.
Main Duties of the Job- Deliver advanced clinical assessment, diagnosis and care planning in clinics, care homes and patients own homes
- Work as part of an MDT coordinating neighbourhood care in line with the Single Neighbourhood Team contract and PCN DES.
- Drive proactive care and admission avoidance
- Ensure KPI delivery and high-quality data recording

Job DescriptionJob ResponsibilitiesThe Frailty ACP will provide advanced clinical leadership and care for patients within the Complex Care Cohort, delivering proactive, coordinated, and personalised care in line with SNH service requirements.
Clinical Leadership- Provide leadership for frailty and complex care delivery to the multidisciplinary team
- Support integrated neighbourhood working across services
- Ensure continuity of care and escalation pathways
- Work as part of the PCN MDT and attend/chair wider MDT meetings with other agencies
- Provide supervision and support to team members
- Ensure coordinated care planning and shared decision-making
- Provide acute and chronic clinical care for care home residents in a ward round format
- Ensure compliance with the requirements of the Enhanced Health in Care Homes contract

- Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs
- Comprehensive Geriatric Assessments (CGA)
- Develop and oversee personalised care plans and ReSPECT
- Manage deterioration and support admission avoidance
- Provide support to patients and families at end of life
- Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan
- Initiate and interpret tests and investigations as appropriate.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
- Collaborate with other members of the Primary Health Care Team including doctors, nurses and AHPs, accepting referrals and referring to them for specialist care.
- Direct referral to and liaison with consultants and other professional agencies.
- Ensure a high standard of clinical practice is maintained by ensuring care is based on current best practice, embarking upon further training if required and continuing to update clinical knowledge.
- Recognise and work within own competence and professional code of conduct as defined by their statutory regulator (HCPC/NMC)
- Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures
- Be aware of data protection (GDPR) and confidentiality issues
- Prioritise, organise and manage own workload in a manner that maintains and promotes quality
- Keep up to date with current evidence-based practice.
- Participate in research and utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required
- In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate
- Support and participate in shared learning across the practice and wider organisation
- Assist in the development and implementation of policies and procedures
- Understand and apply legal issues that support the identification of vulnerable and abused adults and be aware of statutory vulnerable patients health procedures and local guidance.
Person SpecificationExperienceEssential- Minimum three years post-registration experience
- Advanced level of clinical practice within current scope of practice
- Able to understand and analyse complex issues and balance competing priorities to make difficult decisions
- Understanding of NHS principles and values
- Experience of working in a Primary Care setting
Desirable- Supervisory experience
- Mentorship of peers and students
QualificationsEssential- Nurse, paramedic or other registered healthcare professional, registered with the HCPC/NMC as appropriate, with no restrictions on practice or pending Fitness To Practice concerns
- Completed, or on ACP Masters Pathway with progression the pathway
- Full UK Driving License
- Independent Prescriber
- Able to work independently without direct supervision
Desirable- Frailty experience
- Full ACP Masters qualification
Knowledge CriteriaEssential- IT competency, particularly in Windows and Office suite
