Job summary
Brent and Harrow Community Diabetes Services

Central London Community Healthcare NHS Trust are seeking an enthusiastic and experienced GP with Special Interest (GPwSI) in Diabetes to join the Brent and Harrow Community Diabetes Services. The post will support the delivery of high-quality, patient-centred diabetes care within the community, working closely with primary care, community and specialist diabetes teams.

The successful candidate will provide specialist clinical assessment and management for people living with diabetes, with a particular focus on complex or poorly controlled diabetes and supporting the optimisation of treatment. The role will also contribute to multidisciplinary working, clinical advice and education for primary care colleagues, and the development of integrated diabetes pathways across Brent and Harrow.

This is an excellent opportunity for a GP with a strong interest in diabetes to develop their specialist skills while contributing to improving outcomes and reducing health inequalities for people living with diabetes across the local community.

Main duties of the job
The postholder will provide specialist clinical advice, guidance and support to colleagues and patients, ensuring high standards of diabetes care. The role will focus on improving patient outcomes, reducing avoidable emergency hospital admissions, promoting effective self-management and enhancing the quality of life of patients with complex diabetes needs.

Working collaboratively within the multidisciplinary community team, the postholder will support the delivery of high-quality, evidence-based diabetes management, treatment and education for patients receiving care across Brent and Harrow Diabetes Services.

The postholder will also provide clinical guidance and support to nursing and Allied Health Professional (AHP) colleagues, working under the supervision of the Community Consultant Diabetologist.

In addition, they will actively contribute to multidisciplinary team meetings, supporting coordinated care planning, effective clinical decision-making and continuous service improvement.
