Central London Community Health Trust
Rehabilitation Assistant
The closing date is 11 November 2026
Job summary
An exciting opportunity has arisen for a compassionate and motivated Rehabilitation Support Worker to join our friendly and supportive Community Rehabilitation Team on a 7-month fixed-term contract until 31 March 2027.

Working under the supervision of registered clinicians, you will provide essential support to a multidisciplinary team, including Physiotherapists, Occupational Therapists and Speech and Language Therapists, delivering rehabilitation programmes that enable patients to maximise their independence within the community.

You will support patients with a range of physical, cognitive and psychological needs, delivering interventions guided by individual rehabilitation plans to help them achieve their goals and maintain independence in their own homes.

This is an excellent opportunity for someone who enjoys working as part of a multidisciplinary team and is passionate about delivering high-quality, person-centred care within the community.

Please note: This is a fixed-term contract until 31 March 2027 and is not eligible for Skilled Worker visa sponsorship.
Main duties of the job
As a Rehabilitation Assistant, you will be expected to:
  • Support therapy home visits and community-based assessments alongside registered clinicians, or independently once you have achieved the required competencies.
  • Deliver delegated therapy interventions and rehabilitation programmes, following agreed care plans and instructions from the supervising clinician.
  • Monitor patients during treatment, recognise changes in their condition or signs of deterioration, and promptly report and document concerns to the registered clinician and MDT.
  • Promote patient safety and quality of care, including supporting falls prevention and reducing the risk of pressure ulcers.
  • Communicate clearly and sensitively with patients, families, carers and the MDT, using negotiation and conflict-resolution skills when faced with challenging behaviour.
  • Coordinate therapy appointments, manage session bookings and liaise with healthcare professionals to support timely, joined-up care.
  • Guide carers, families and education staff in understanding and carrying out prescribed therapy programmes.
  • Seek supervision and clarification when managing complex cases, unfamiliar tasks or unclear instructions.
  • Work safely and professionally in varied community environments, including homes affected by poor hygiene, pets or clutter.
  • Remain flexible, organised and calm under pressure, adapting to changing caseload demands, priorities and stressful situations.
