Channel View Medical Group is looking to recruit an experienced Advanced Practitioner (ANP/ACP) to join our friendly and supportive multidisciplinary team. We welcome applications from both Advanced Nurse Practitioners (ANPs) and Advanced Clinical Practitioners (ACPs) who are committed to delivering safe, evidence-based care and making a real difference to our patients.
This is an exciting opportunity to work autonomously within a progressive general practice, providing expert clinical care while contributing to service development, education and quality improvement.
Main duties of the jobThe Advanced Practitioner will work autonomously within their professional scope of practice, providing high-quality, patient-centred care through face-to-face, telephone and digital consultations as appropriate.
The post holder will assess, diagnose, treat and manage patients presenting with acute, chronic and undifferentiated conditions, undertaking comprehensive history taking, clinical assessment, diagnosis, treatment, evaluation of care and referral for investigations or specialist services where required.
Working alongside GPs and the wider multidisciplinary team, the successful candidate will demonstrate advanced clinical decision-making, expert assessment skills and sound clinical judgement to deliver safe, effective and evidence-based care.
The role also includes contributing to clinical leadership, education, service development and quality improvement, supporting the delivery of excellent patient care and the continued development of the practice.

Job responsibilitiesAbout youWe are looking for an enthusiastic and motivated Advanced Practitioner who:
- Is a registered healthcare professional with the appropriate UK regulatory body (e.g. NMC, HCPC or GPhC).
- Holds a recognised Master's level Advanced Clinical Practice qualification (or equivalent advanced practice qualification).
- Is an Independent Prescriber (or working towards where appropriate).
- Has experience of working autonomously in primary care, urgent care or a similar clinical setting.
- Has experience assessing and managing patients with acute illness, minor illness and injury, long-term conditions and undifferentiated presentations.
- Demonstrates excellent communication, leadership and clinical decision-making skills.
- Is committed to continuous professional development, evidence-based practice and delivering outstanding patient care.
Key ResponsibilitiesClinical Practice- Provide autonomous assessment, diagnosis, treatment and management of patients presenting with acute, chronic and undifferentiated conditions.
- Undertake comprehensive clinical assessment, formulate management plans and evaluate patient outcomes.
- Request, interpret and act upon appropriate investigations.
- Prescribe safely and effectively within scope of practice as an Independent Prescriber.
- Refer patients appropriately to secondary care, community services and other healthcare professionals.
- Manage long-term conditions and support patients to improve their health and wellbeing through prevention and self-management.
- Respond appropriately to urgent and emergency presentations.
Leadership, Quality and Service Development- Contribute to clinical leadership and the ongoing development of practice services.
- Participate in audit, quality improvement, clinical governance and risk management activities.
- Support the development and implementation of evidence-based clinical policies and pathways.
- Contribute to achieving national and local quality standards and contractual requirements.
Communication and Team Working- Work collaboratively with GPs, nurses, pharmacists, allied health professionals and the wider multidisciplinary team.
- Build effective relationships with patients, carers and external organisations to deliver coordinated care.
- Maintain accurate, timely and comprehensive clinical records using the practice clinical system.
- Communicate effectively with patients and colleagues, adapting communication to individual needs.
Professional Responsibilities- Maintain professional registration and practise within the relevant professional code and scope of practice.
- Maintain competence through continuing professional development, appraisal and revalidation.
- Participate in education, supervision and the support of colleagues and learners where appropriate.
- Comply with all practice policies relating to safeguarding, information governance, infection prevention, health and safety, equality and diversity and clinical governance.
The role requires the post holder to demonstrate the four pillars of Advanced Clinical Practice by:- Delivering expert autonomous clinical care.
- Supporting education and development of colleagues.
- Contributing to leadership and service improvement.
- Promoting evidence-based practice, audit and quality improvement
