We are a well-established, forward-thinking GP practice caring for a patient population of over 14,000 patients. Main duties of the jobThe successful candidate will provide advanced clinical assessment, diagnosis, treatment, and management of patients presenting with both acute and chronic conditions within primary care. Key responsibilities include:
- Independently assess, diagnose, treat, and manage patients within your scope of practice.
- Prescribe medication as an Independent Prescriber.
- Request, interpret, and act upon appropriate investigations.
- Manage undifferentiated and acute presentations.
- Refer patients appropriately to secondary care and community services.
- Promote health, wellbeing, and self-management.
- Work collaboratively with GPs and the wider multidisciplinary team.
- Maintain accurate and contemporaneous clinical records.
- Contribute to clinical governance, audit, and service improvement.

Job responsibilitiesMain responsibilities: The Advanced Nurse Practitioner will:
- Deliver advanced clinical practice in accordance with NMC standards and national guidance.
- Assess, diagnose, treat, and manage patients presenting with acute and chronic conditions.
- Independently prescribe and review medication.
- Request and interpret investigations.
- Make evidence-based clinical decisions.
- Refer patients appropriately to internal and external services.
- Deliver health promotion and preventative care.
- Work closely with the multidisciplinary team to ensure continuity of care.
- Participate in clinical audit, quality improvement, and service development.
- Maintain professional registration and undertake continuing professional development.
- Support the education and development of colleagues where appropriate.
Person SpecificationExperienceEssential- Registered Nurse with current NMC registration.
- Advanced Clinical Practice qualification (Master's level or equivalent).
- Independent Prescriber.
- Post-registration experience.
- Experience working autonomously within primary care, urgent care, or a similar setting.
- Advanced clinical assessment and diagnostic skills.
- Experience managing acute and long-term conditions.
- Excellent communication and interpersonal skills.
- Sound clinical judgement and decision-making.
- Ability to work independently and collaboratively.
- Excellent organisational and IT skills.
- Commitment to delivering outstanding patient-centred care.
Desirable- Previous General Practice experience.
- Experience using SystmOne.
- Minor illness/minor injury qualification.
- Experience in quality improvement and clinical audit.
QualificationsEssential- Must be a Registered Nurse with active registration on the Nursing and Midwifery Council (NMC) register.
