Job summary
We are seeking an enthusiastic, experienced and highly motivated Home Visiting Practice Nurse to join our Primary Care Network (PCN). This is an exciting opportunity to deliver high-quality, patient-centered care to individuals within their own homes, supporting some of our most vulnerable and complex patients.

Working autonomously across the PCN, you will provide comprehensive nursing care, chronic disease management, vaccinations, health promotion and holistic assessments to patients who are housebound or unable to attend GP practice appointments.

You will play a key role in improving patient outcomes, reducing health inequalities and supporting proactive, preventative care within the community.

Main duties of the job
  • Conduct home visits to patients across the PCN.
  • Deliver evidence-based care for patients with long-term conditions, including:
    • Diabetes
    • Chronic Obstructive Pulmonary Disease (COPD)
    • Asthma
    • Hypertension
    • Cardiovascular disease
    • Chronic kidney disease
  • Undertake chronic disease reviews and monitoring in accordance with local and national guidelines.
  • Administer vaccinations and immunisations, including seasonal vaccination programmes.
  • Complete comprehensive holistic assessments, identifying physical, psychological and social care needs.
  • Develop and deliver personalised care plans.
  • Provide health promotion, self-management advice and preventative interventions.
  • Monitor and manage patients with complex health needs in collaboration with multidisciplinary teams.
  • Undertake clinical observations and assessments.
  • Identify deteriorating patients and escalate concerns appropriately.
  • Maintain accurate and contemporaneous clinical records using electronic patient record systems.
  • Participate in safeguarding processes and ensure patient safety at all times.
  • Liaise effectively with GPs, community nursing teams, social care services, secondary care providers and voluntary sector organisations.

About us
Chester Central Primary Care Network (PCN) is a collaboration of six GP practices located in the center of Chester, caring for a population of approximately 41,000 patients.

The practices are based in three locations, within walking distance of each other, four are based in the Fountains Health Building and we have the unique opportunity of the homeless practice within our PCN.

We are a friendly team with a number of additional roles (ARRS) including a wellbeing/social prescribing link worker, dieticians, GPs, clinical pharmacists, phlebotomist, paramedics and many more who work alongside our practice teams.

We are an inclusive PCN and have strong working relationships within the practices, ICB and external organisations.

Job responsibilities
The Home Visiting Practice Nurse will work as part of the Primary Care Network multidisciplinary team, delivering high-quality, patient-centred nursing care to housebound patients and those unable to access services within a GP practice setting.

The post holder will work autonomously across the PCN, undertaking home visits to provide chronic disease management, vaccinations and immunisations, health promotion, preventative care and holistic patient assessments. They will play a key role in supporting proactive care, reducing health inequalities and improving health outcomes for patients within their own homes.

The role requires a highly skilled and experienced nurse who is confident working independently, managing clinical risk and making evidence-based decisions whilst collaborating effectively with the wider healthcare team.

Person Specification
Essential
  • Registered Adult Nurse with current NMC registration.
  • Evidence of continuing professional development.
  • Recognised training in vaccinations and immunisations.
  • Experience working within Primary Care, Community Nursing or a similar setting.
  • Experience managing patients with long-term conditions.
  • Experience undertaking clinical assessments and patient reviews.
  • Experience delivering vaccination programmes.
  • Experience of working independently and managing a caseload.
  • Excellent clinical assessment and decision-making skills.
  • Sound understanding of chronic disease management.
  • Ability to provide holistic, patient-centred care.
  • Knowledge of safeguarding adults and vulnerable patients.
  • Understanding of clinical governance and risk management.
  • Excellent communication and interpersonal skills.
  • Ability to prioritise workload effectively.
  • Competent IT skills and experience using electronic patient record systems.
  • Ability to work autonomously whilst recognising limitations and seeking support when required.

Desirable
  • Independent Nurse Prescriber qualification.
  • Long-term condition management qualification.
  • Mentorship or teaching qualification.
  • Experience working within a Primary Care Network.
  • Experience caring for frail, housebound and complex patients.
  • Experience participating in quality improvement projects.
  • Understanding of personalised care and population health management.
  • Knowledge of local health and social care services.
  • Understanding of NHS Primary Care Network objectives and priorities.
