Job summaryWorking with our community nursing team, you'll coordinate and deliver personalised care that helps people stay well and remain independent at home. You'll play a key role in supporting patients with complex health needs, long term conditions and palliative care, helping to improve outcomes and reduce avoidable hospital admissions. As a Case Manager, you will manage a defined caseload and work closely with patients, carers, General Practitioners, community services and other healthcare professionals. You will use your clinical expertise to assess needs, develop care plans and provide high-quality nursing interventions, while supporting colleagues and promoting best practice.
Main duties of the jobWhat you'll do:- Assess, plan, deliver and review care for patients with complex nursing needs
- Coordinate care and work with partners across health and care services
- Provide specialist nursing interventions and support patient self-management
- Manage and prioritise a caseload while maintaining accurate electronic care records
- Support and develop colleagues through supervision, guidance and shared learning
What we're looking for:- Registered Nurse qualification with current professional registration
- Experience caring for people with long term conditions and complex health needs
- Strong assessment, communication and care coordination skills
- Ability to work independently and as part of a multidisciplinary team
- Commitment to delivering safe, person-centred, evidence-based care and continuous professional development

Person SpecificationQualificationsEssential- RGN/RN/Diploma Nursing Studies
- Non-medical prescribing V100/V150 or willing to work towards within specified timeframe
- Specialist training/short courses relevant to the post or equivalent level of experience
- Evidence of ongoing continuing professional development/Portfolio
Desirable- Specialist Community District Nursing post graduate diploma
KnowledgeEssential- Current Legislation and impact on community nursing services.
