Job summary
We are looking for an enthusiastic and dynamic, and experienced Band 3 Generic Support Worker to join the well-established Hull East riding Falls service. The post holder will also be expected to work as part of the wider multidisciplinary falls team to provide Therapy and Nursing support to patients/clients within standard operation procedures and contribute to the care planning process, this will include travelling across both hull and East Riding to visit patients in their own homes. You will support therapists, nurses and other healthcare professionals by carrying out delegated tasks, monitoring progress and helping services run smoothly. Your work will make a real difference to patients recovering from illness, injury or a loss of independence. What you'll do:

• Support therapy and nursing programmes for patients following agreed care plans
• Record observations and report any changes in a patient's condition
• Help patients use equipment safely and support their rehabilitation goals
• Work with healthcare professionals, carers and external agencies to support patient care
• Assist with administrative tasks and contribute to the efficient running of the service

Main duties of the job
What we're looking for:

• A Level 3 in Health and Social Care or equivalent knowledge and experience
• Good communication, organisation and teamworking skills
• An understanding of confidentiality and person-centred care
• Experience supporting people with physical or mental health needs, or transferable skills from a similar setting
• The ability to work independently within agreed guidelines and as part of a wider team

Qualifications
Essential

• Level 3 in Health and Social Care or equivalent relevant knowledge and experience

Knowledge & Exprience

• Working knowledge of relevant policy, legislation drivers and their application to clinical and service area e.g. Mental Capacity Act, Social Inclusion
• Basic IT skills
• Understanding of confidentiality and information governance
• Appropriate knowledge/experience of working with people with physical and/or mental health problems

• Willingness to undertake further academic development
• Knowledge of multi-disciplinary working
• Experience of working in a community setting
• Experience of moving and handling people
