Job summary
We are seeking a motivated and compassionate Registered Nurse to join our Out of Hours Nursing Team, delivering high-quality unplanned care to patients across the Hull and East Riding area.

In this rewarding role, you will assess, plan, and provide nursing care to patients within their own homes and community settings, helping to meet urgent healthcare needs outside of normal working hours.

Main duties of the job
What you'll do:

• Assess patients' needs and deliver safe, person-centred nursing care in line with care plans.
• Triage, assess and provide unplanned care to support patients in the community and help prevent hospital admissions
• Work with multidisciplinary teams and partner organisations to coordinate effective care and referrals
• Maintain accurate clinical records and contribute to high standards of patient safety and care quality
• Support learning and development by sharing knowledge with colleagues, students and carers

Essential requirements:

• Registered Nurse with current NMC registration
• Excellent clinical assessment and communication skills
• Commitment to delivering patient-centred care
• Ability to work independently and as part of a multidisciplinary team
• Full UK driving licence and access to a vehicle suitable for the role

Qualifications
Essential

• Degree in Nursing Studies
• First level Registered Nurse or working towards final assessment to achieve first level registration

• Demonstrate an understanding of the role of a community nurse
• Knowledge of legislation impacting on service delivery
• Knowledge of the modernisation agenda
• Demonstrate an understanding of evidence based practice

• Demonstrate an understanding of the MDT
• Basic IT skills
• Knowledge and skills in the assessment process

Experience
