Job summary
Working with a supportive nursing team, you'll deliver high-quality care that helps patients recover, maintain independence and achieve the best possible outcomes. Based within Integrated Community Services, you will play an important role in providing safe, compassionate and person-centred care while supporting colleagues and contributing to a positive patient experience. You'll assess patient needs, plan and deliver care, and help ensure services run smoothly during each shift.

Main duties of the job
What you'll do:

• Assess, plan, deliver and evaluate nursing care for a group of patients
• Provide safe, compassionate care and respond to changing patient needs
• Support and guide junior staff, learners and students in daily practice
• Maintain accurate patient records and follow clinical governance processes
• Help coordinate shifts and contribute to the smooth running of the service when senior staff are unavailable

What we're looking for:

• Registered General Nurse (RGN) or Registered Nurse qualification
• Recent experience working in a nursing role
• Good communication and leadership skills
• An understanding of multidisciplinary team working and evidence-based practice
• A flexible approach and willingness to continue learning and developing your skills

Qualifications
Essential

• RGN/RN/Diploma Studies
• Be willing to undertake further training and development

Desirable

• Completion of post qualification preceptorship

Knowledge
Essential

• To have recently worked within a nursing capacity
• Demonstrate an understanding of the MDT

Desirable

• Community Hospital experience
• Demonstrate an interest in palliative care
• Demonstrate an understanding of evidence based practice
• Have an interest in audit and maintaining standards

Experience
Essential

• Good communication and leadership skills
• Be able to provide education to members of the ward team
• Demonstrates an awareness of own and others competencies
