Job summaryINTERVIEW DATE THURSDAY 22ND OCTOBER 2026This is an exciting opportunity for the right candidate to join our existing team of Pharmacists and Pharmacy Technicians, working within the GP Surgeries that form Wakefield North Primary Care Network (PCN). The current team, led by our Lead Pharmacist and Lead Pharmacy Technician, have built strong and valued working relationships amongst themselves, other roles within the PCN and our GP practices as well as colleagues from a neighbouring PCN.
Main duties of the job- Manage repeat prescription processes, including resolving queries, maintaining accurate medication records, and working from prescribing ledgers in a timely manner.
- Undertake medicines reconciliation following discharge or correspondence from secondary care, community services, and clinics.
- Maintain and optimise repeat medication lists, including synchronisation, review date management, electronic repeat dispensing, and appropriate amendments under agreed protocols.
- Support care home and nursing home patients through medicines optimisation, prescription changes, and liaison with clinicians and community pharmacies.
- Deliver patient-facing clinics where appropriate, including medicines optimisation, secondary prevention, cardiovascular disease (CVD) support, and frailty reviews.
- Undertake clinical support activities including observations (e.g. blood pressure), urine dip testing and, if required, phlebotomy (training provided).
- Support antimicrobial stewardship, opioid de-prescribing, safety alerts, and the implementation of national and local prescribing guidance.
- Contribute to prescribing safety through audits, quality improvement activity, monitoring of recalls, blood tests, and medication reviews.
- Support effective stock management, prescribing data analysis, cost-effective prescribing, and waste reduction initiatives.
- Promote safe use of medicines, report and prevent medicines-related incidents, and escalate safeguarding concerns appropriately.

Job responsibilitiesThe Pharmacy Technician is an integral member of the general practice team, working within professional boundaries across practices within the PCN. The role operates under the supervision of the PCN Clinical Pharmacists to support the safe, accurate and timely use of prescribed medicines.
The postholder will provide technical and clinical support to Clinical Pharmacists and clinicians with the practices, including responding to medication-related queries, undertaking medicines reconciliation following care transitions and supporting prescribing safety through audits and data presentation. The role may also involve patient-facing work, including the delivery of technician-led clinics where appropriate.
Pharmacy Technicians play a key role within the PCNs multidisciplinary team, contributing across clinical, technical, and administrative functions to support effective medicines optimisation and improve patient outcomes.
QualificationsEssential- Educated to GCSE Mathematics and English Grade C or above level or equivalent NVQ level 3 in pharmacy services or equivalent. Registration with the General Pharmaceutical Council GPhC Evidence of Continued Professional Development Excellent time management skills Excellent communication skills, demonstrating clear and polite verbal skills Adaptable and quick to make relationships with new teams Flexible and cooperative Ability to identify risk and escalate appropriately Ability to maintain confidentiality Evidence of working autonomously or as part of a team Holds a full driving licence with access to a vehicle to enable travel across the PCN area and Wakefield District Disclosure Barring Service DBS check
Desirable- Completion of CPPE primary care educational pathway. Experience working in a primary care setting / PCN Knowledge of General Practice repeat prescribing systems Use of SystmOne
