Are you a compassionate, motivated and adaptable Registered Nurse looking to make a real difference in your local community? We are seeking enthusiastic Community Nurses to join our friendly and supportive Launceston and Tamar Valley Community Nursing Team.

Working across the Callington and Launceston areas, you will be part of a dedicated multidisciplinary team delivering high-quality, person-centred care.

You will assess, plan and provide care for patients with a range of health needs, promoting independence, wellbeing and self-management. Working closely with GPs, therapists, health and social care professionals, and voluntary organisations, you will help people remain safe and well within their communities.

We are looking for nurses who are passionate about patient care, confident working independently, enjoy teamwork, and are adaptable, resilient and committed to continuous learning and development.

This role requires regular travel between patient homes, clinics and community settings. Due to Cornwall's rural geography and limited public transport, access to suitable transport for work-related travel is essential.

Main duties of the job
Community Nursing operates seven days a week between 08:00 and 22:00, with a range of shift patterns including 08:00-16:00, 08:00-18:00, 12:00-22:00 and 14:00-22:00. Through a comprehensive induction programme and ongoing clinical supervision, you will develop the skills and confidence to deliver high-quality care within patients' homes and community settings.

You will assess, plan, implement and evaluate care for patients, carers and families, promoting independence, wellbeing and quality of life. Responsibilities include undertaking holistic assessments, developing personalised care plans, providing health education and support, and delivering evidence-based nursing care. You will work autonomously within your scope of practice while collaborating closely with GPs, therapists, social care teams and other partners to provide seamless care.

As part of a flexible service, nurses may work across locality teams to meet service needs, offering opportunities to broaden experience, develop new skills and work alongside a variety of professionals while delivering outstanding community care.

Person Specification
Education and Qualifications
Essential

• Registered Nurse (Adult)
• Evidence of recent relevant professional development
• To act as a mentor for student university placements. Having the mentoring qualification or being willing to work towards this qualification and attending mentorship updates as required.
• Current Professional Registration - NMC

Desirable

• Hold a post-registration diploma or be working towards a degree

Skills and Aptitude
Essential

• IT skills to include use of Word, Excel, email, internet, recording systems
• The ability to provide skilled, evidence based nursing care to a wide range of patients
• Ability to deal with patients and carers who are under stress.
• Awareness of and adherence to policies including lone worker and manual handling policies
• Current knowledge of developments in nursing and healthcare in general.
