Job summary
A post has arisen for a band 6 to work as part of the Diabetes Specialist Nursing Team. The successful candidate will work to enhance their diabetes skills and provide advanced assessment and support to the whole health community. We are looking for a registered nurse who is enthusiastic about the field of diabetes, wanting to improve your knowledge and make a difference of patients in the community.

Previous diabetes expertise is essential with proven evidence of working within a Diabetes Team.

Main duties of the job
The post holder will provide expert specialist nurse led and intermediate clinics and support staff, patients and carers. Managing an on-going patient case load of people with Diabetes using nurse led clinics, GP cluster clinics, ward reviews in community and acute hospital settings and domiciliary visits, throughout the whole patient pathway; working towards agreed standards and targets as determined by the DSN lead.

To assist with the professional leadership and management of the Cornwall Adult Diabetes Nursing Service in conjunction with Clinical Nurse Specialist Diabetes and Lead Clinical Nurse Specialist to provide the highest possible standards of care to the patients and their families

Person Specification
Education/Qualification and Relevant Experience
Essential
  • Current professional registration - NMC
  • Professional qualification - RN level 1
  • Diabetes specialist course (ENB 928 or Diabetes degree module) or willingness to undertake
  • Non- Medical Prescribing qualification or willingness to undertake
  • Substantial Post Registered (RN) experience.
  • Employed at a senior level within an adult care acute / community setting caring for people with Diabetes
  • Evidence of continuous professional development

Knowledge and Abilities
Essential
  • Knowledge of current issues in Diabetes care
