Job summaryAn exciting opportunity has arisen for an enthusiastic and motivated Physical Health & Wellbeing Nurse to join our Community Mental Health Rehabilitation Team, based at St Austell Hospital.
The team provides county-wide support for people recovering from complex and enduring mental health difficulties, helping individuals transition from inpatient rehabilitation services to independent living within the community. Working alongside Fettle House and a range of partner agencies, including carers, families, housing providers, education services, pharmacy teams and specialist mental health services, we deliver person-centred rehabilitation and recovery-focused care.
The successful candidate will play a key role in promoting and monitoring the physical health of people on the caseload. Responsibilities include undertaking annual physical health checks, liaising with GPs regarding ongoing health monitoring and follow-up care, and supporting individuals to improve their overall health and wellbeing.
You will contribute to the development of personalised care plans, support and supervise junior staff, and participate in the ongoing review and improvement of the rehabilitation service. Regular supervision and professional support are provided.
This is a part-time position working 09:00-17:00, with days negotiable.
Main duties of the jobWe have an exciting opportunity for a dynamic and passionate nurse to join our Community Mental Health Rehabilitation Team, using their holistic nursing skills to improve the physical health and wellbeing of adults accessing mental health services.
Working closely with patients, families and multidisciplinary colleagues, you will promote high standards of physical healthcare across community services. You will undertake holistic assessments, identify and manage physical health needs, develop and review care plans, and ensure care is safe, effective and evidence based.
You will support Mental Health Nurses and Healthcare Assistants to confidently deliver physical healthcare, providing guidance, supervision and training where needed.
The role also provides opportunities to contribute to audits, CQUINs, health and safety initiatives, service development and quality improvement projects. You will be encouraged to identify and implement improvements that positively impact patient care, outcomes and experience.
As this is a community-based role, regular travel across the county is required, including to locations that may not be accessible by public transport. Candidates will need to demonstrate how they can meet these travel requirements.

Person SpecificationEducation / Qualifications and Relevant ExperienceEssential- Professional qualification - Registered General Nurse (adult) or Dual Registered RGN/RMN or RMN with additional skills in general nursing
- Post graduate training or equivalent relevant experience
- Mentorship module or willingness to undertake
DesirableKnowledge and abilitiesEssential- Ability to provide physical health care in mental health community bases and in the patients own home environment
