Job summaryAn exciting opportunity has arisen for a Band 5 Physical Health Nurse to join our established multidisciplinary Healthcare Team as we expand into the new Millennium Lodge Secure Children's Home in Sleaford. The team provides comprehensive healthcare to vulnerable children and young people aged 10-18 who may present with complex physical health, mental health, neurodevelopmental, substance misuse and psychosocial needs.
Working alongside our Band 6 Physical Health Practitioner and wider MDT, you will contribute to physical health assessment and screening, clinical observations, health promotion, long-term condition monitoring, immunisation and vaccination programmes, sexual health interventions and primary care clinics. We are looking for a compassionate, motivated Registered Nurse committed to child-centred, non-stigmatising and trauma-informed care.
The service operates 09:00-21:00 Monday-Friday and 09:00-13:00 weekends and Bank Holidays. The role includes early and late shifts and participation in the on-call rota for new admissions during commissioned weekend and Bank Holiday hours.
Main duties of the jobYou will deliver direct physical healthcare to children and young people within Millennium Lodge as part of the multidisciplinary Healthcare Team. You will undertake physical health assessments, screening and clinical observations; recognise and appropriately escalate abnormal findings or deterioration; contribute to individual healthcare plans; and undertake clinical procedures within their competence.
The role will support immunisation and vaccination programmes, sexual health interventions, health promotion, long-term condition monitoring and visiting GP/primary care clinics. You will work collaboratively with the Band 6 Physical Health Practitioner, GP, wider MDT, residential colleagues and external healthcare providers to ensure continuity of care. You will maintain accurate clinical records, contribute to safeguarding, audit, quality improvement and supervision, and deliver care using a child-first, trauma-informed and non-stigmatising approach.
You will work flexibly across commissioned hours, including early and late shifts, and contribute to the weekend and Bank Holiday on-call rota for new admissions.

QualificationsEssential- Registered Nurse with current NMC registration
- Evidence of continuing professional development
- Willingness to undertake additional training and competency development relevant to the physical healthcare needs of children and young people within a secure setting
Desirable- Registered Adult Nurse or Registered Children's Nurse
- Additional training relevant to physical healthcare, young people's health, sexual health, immunisation or long-term conditions
- Practice supervisor/ assessor preparation or willingness to undertake this following appropriate experience
ExperienceEssential- Experience of delivering direct nursing care
