Job summary
Exciting new opportunity for a permanent part time specialist physiotherapist to join our Community Stroke and Neurology Rehabilitation Service. The post is based in West or Mid Cornwall depending on level of clinical demand. The service provides specialist assessment and rehabilitation for patients with a wide variety of complex neurological conditions, including Stroke. This friendly, enthusiastic and dynamic team are aiming to expand, increasing therapy clinics and have access to pool cars for community visits. There are opportunities for this post to become rotational in the future. You'll be working with experienced health care professionals as part of an integrated multidisciplinary team who will give support, joint working, regular supervision, appraisal, in-service training programme and a real commitment to professional development for you to reach your full potential.The ideal candidate will have experience working with stroke or neurological conditions, rehabilitation experience, excellent communication skills, team working, time management, and a flexible approach. Duties will include completing specialist neurological and functional assessments, formulating, and implementing rehabilitation programmes, supporting patient self management, education and support of carers, wider health, social care and voluntary services involved in patient care.

Main duties of the job
To be an autonomous practitioner independently managing a caseload of patients with complex stroke and neurological conditions, formulating diagnosis and treatment plans.Specialist knowledge in the specific area of stroke, neurology and eldercare.Implementing and evaluating rehabilitation programmes using appropriate outcome measuresTo provide education, and supporting condition management for patients, carers and multidisciplinary team.To support education and development of wider services, in management of stroke and neurological conditions.Excellent communication and interpersonal skills.Effective and supportive team working with extensive experience of collaborative multidisciplinary team working.To support service development, audit and engagement in research.Main accurate and comprehensive patient treatment records in line with CSP and organisational record keeping standardsGood IT skills.

Person Specification
Education/Qualification
Essential
  • Current professional registration HCPC
  • Evidence of further formal study eg. Post graduate qualifications (Postgrad Certificate/Diploma/MSc)
  • Membership of the Chartered Society of Physiotherapy

Desirable
  • Clinical Placement Supervisor training

Experience
Essential
  • Act as an autonomous practitioner formulating diagnosis and treatment plans.
  • Have acquired experience and knowledge in eldercare, orthopaedic, stroke, neurological and respiratory conditions; community rehabilitation and specialist rehabilitation clinics, falls prevention, intermediate care and younger patients with chronic disability.
  • Evidence of broad range of experience in stroke/ neurology
  • Highly developed skills in clinical assessment, intervention and evaluation
  • Extensive experience of working within a multidisciplinary team
  • Proven experience in managing a change process.

Desirable
  • Basic understanding of and ability to apply skills in leadership and management with evidence of taking the lead /a lead role in a project.
  • Proven experience of managing a service.

Skills and Aptitude
Essential
  • Able to work well in a team
  • Commitment to client centred, non-discriminatory practice.
  • Able to engage individuals in the therapeutic process.
  • Experience of supervising people including students, qualified and support staff.

Knowledge and abilities
Essential
  • Specialist knowledge and application of Physiotherapy assessments and interventions.
  • Able to apply client centred approach and principles.
  • Able to work in highly stressful situations including the ability to manage distressing and highly emotive situations successfully (eg. Working with people recently diagnosed with life changing conditions).
  • Ability to work flexibly in a range of settings eg outpatient departments, hospital wards, GP surgeries and in the patient's own home.

Desirable
  • Good Leadership skills
  • Presentation skills and ability to teach others to a countywide, multidisciplinary audience.
